PURPOSE: ENTER INTO AGREEMENT WISH WINNEBAGO COUNTY TO
PROVIDE HEALTH SERVICES
INITIATED BY: HEALTH DIVISION
WFiEREAS, the City is eligible to receive funds through the State
o£ Wisconsin's Division of Health block grant funding process; and
WHEREAS, funding levels for the City of Oshkosh, based on a
formula allocation, are estimated at $10,609 for Maternal and Child
Health (MCH) and $5, 622 for Prevention; and
WHEREAS, it would be advantageous for the City to contract with
another agency to provide certain health services under said
programs for citizens within the City of Oshkosh; and
WHEREAS, the Winnebago County Public Health Department has
expressed an interest in providing said health services in the form
of a Refugee Health Services program and an Adolescent Parent
Education and Support Group program for citizens within the City of
Oshkosh; and
WHEREAS, services to be provided by Winnebago County shall not
exceed funding levels received by the State oP Wisconsin.
� NOW, THEREFORE, BE ZT RESOLVED by the Common Council of the City
of Oshkosh that said health services to be provided by the Winnebago
County Public Health Department are hereby approved, contingent upon
final approval by the State of Wisconsin Division of Health.
BE IT FURTHER RESOLVED that t�e proper City officials are hereby
authorized to enter into ane`�ae� with the Winnebago county Public
Health Department for said services, and any and all documents
required by the State of Wisconsin Division of Health.
