This form is required to be submitted by the supervising professional (architect, engineer, HVAC designer or electrical
designer) observing construction of projects within buildings with total areas exceeding 50,000 cubic feet or greater and
bleachers (Comm 50.10/Comm 61.50). Failure to submit this form may result in penalties as specified in Comm
50.26/Comm 61.23 andlor local ordinances.
General Instructions: Prior to the initial occupancy of new buildings or additions and the final occupancy of

The above-reference plans have been stamped CONDITIONALLY APPROVED based upon
review for confonnance to the current edition ofthe Wisconsin Administrative Building and
Heating, Ventilating and Air Conditioning Code, chapters COMM 61-65 - These plans have
NOT been reviewed for confonnance to the Plumbing Code (chs. COMM 81-86), the Electrical
Code (ch.COMM 16) and any COMM code not specifically mentioned. This approval is not a
Building Pennit. Necessary city pennits must be secured before commencing work.
You are hereby advised that the owner, as defined in Chapter 101.01 (I) of the Wisconsi;n State
Statutes, is responsible for all code requirements not specifically cited herein- Code .
requirements are set forth in Chapters 61 through 65 of the rules of the Department of
Commerce.
The building will be inspected during construction and a final inspection will be made after
completion to insure compliance with city and state codes.
The architect, professional engineer, builder or owner shall keep at the building as evidence of
approval, one set of plans bearing the stamp of approval.
COMM 64.02 This approval does not include heating and ventilating. Such plans are required to
be submitted and approved prior to installation of such equipment.
H & R Block meets the requirements for one exit, therefore emergency illumination is not
required.
Check Into Cash is required to have 2 exits, therefore it must comply with all emergency
illumination requirements-
Be advised, a $100.00 Plan Submittal fee is due. It will be collected from RJ Albright at Pennit
issuance.
