I Hereby propose to furnish all the materials and perform all labor necessary for
the completion of:

Concrete Driveway - 1010 sq./ft.

JOB INCLUDES:
Garage Floor
1.- Excavation and Removal of all concrete - used on site for fill
2. Number 10 Wire Mesh throughout
3.4000 psi concrete mix
4. Rerod as needed by existing concrete - Grade Beam with 2 1/2"
rerod around entire sides, side by side - 10" x 24"
5.7/8" Gravel Fill under concrete ( 4"- 6" ) as needed, this will be
an extra cost to Owner
6. Concrete to be 6" in thickness
7. Concrete CS-309 sealer to applied 1X time
8. Controling Joints to be cut in concrete by power saw, poly
expansion as needed
9. Gravel Fill to be tampered with Wacker power machine
10. Expansion Joint as needed
11. All forming as needed to complete and then remove
12. Contractor to get permits if needed, this will be an extra charge
to owner
13. Contractor will call diggers hotline to check for utilites running
underground or in the way of excavation for driveway
14. Anchor bolts to be applied when installing garage walls
15. Concrete to have a smooth finish
16. Contractor will not be responsible for lawn damage that
happens
17. Clean up of all materials upon completion
18. Contractor is not responsible for weather ralated problems if
they should occur or time of start or finish date - completion of
