(AECOM) is requesting an amendment to the agreement dated October 14, 2014, AECOM Proposal Number OPP-267826 for Construction Related Services for the Construction related to this project. Pursuant to our meeting on Tuesday, October 13, 2015, we are pleased to offer you the following amendment for Professional Land Surveying services. This amendment is based on your requirement for a Subdivision Plat of the lands contained within the 'Construction Drawings for City Contract 14-10, Southwest Industrial Park Expansion (TID#23) Phases III & IV Utilities'. AECOM appreciates the opportunity to furnish this amendment and it is based on criteria as detailed below. General AECOM will prepare a subdivision plat consisting of Out-Lots, utility easements and public right-of- way dedications consistent with your requirements. Final lot configurations will be developed through the life of this project and the assumption is that there will be 3-5 Out-Lots. Consistent with the referenced Contract, the project will be prepared on the Wisconsin Coordinate Reference System, (WISCRS or WCCS), Winnebago County datum. In order to provide this service, AECOM proposes to complete the following tasks: Research, Scoping and Project Preparation Initial research of records available at the Register of Deeds, via Fidlar, has been completed, (copies attached). Thus far, Deeds and Certified Survey Maps, that will provide the basis for exterior boundary location, have been downloaded and printed. As of today, a thorough search of records at the Winnebago County Courthouse has not been performed and we anticipate performing that research as part of our due diligence. No documents, (provided by you or found during our research), indicate that a current exterior boundary survey of the lands exist. 60334236_Amendment 2.docx AECOM Expansion of the Southwest Industrial Park Construction Contract 14-10, Amendment 2 In conjunction with our research, it is assumed that a current commitment for title insurance will be furnished within three weeks of any authorization to proceed. Estimated Fees: $1,000.00 Prior to platting the lands, an exterior boundary survey must be completed. AECOM will utilize the public records found and provided during the research phase to develop the same. We WILL NOT prepare any survey documents for this work other than filing PLSS Monument Records (if required), and incorporating the boundary into the finai plat. Estimated Fees: $4,300.00 Preliminary Plat Given the unique nature of this proposed development, our assumption is that per Section 30-72 (B) (1) of the City of Oshkosh Municipal Code, (Rev. 12/31/09), the preliminary platting requirement will be waived by the Secretary of the Planning Commission. This proposal does not include any amount for the development of a Preliminary Plat; should one be required, we could prepare a proposal within two days of the request for the same. Final Plat AECOM will prepare a Subdivision Plat consisting of Out-Lots, utility-easements and public road right-of-way dedications consistent with the requirements of the City of Oshkosh. The final number of lots and configurations would be developed during future 'scoping' meetings. The plat will be prepared consistent with the previously referenced Municipal Code and other land division regulation including, but not limited to, Chapter 236 of Wisconsin Statutes and Chapter A-E7 of the Administrative Rules. The quoted fee for this service includes and time and materials necessary for surveying, drafting and monumenting the plat as required. In order to accomplish this goal, we encourage the following workflow along with our anticipated timeline; which assumes an authorization to proceed. • Submittal of Amendment • Acceptance and authorization to proceed • Within 2 weeks of authorization - Additional scoping meeting, (In-person or phone conference) • Within 2 weeks of authorization - Fieldwork commences • Within 3 weeks of authorization — Title Commitment 8� supporting documents provided AECOM • Within 4 weeks of authorization - Boundary completed • Within 6 weeks of authorization - Lot configurations submitted to City for comments, (emaii) • Within 8 weeks of authorization - Final Plat submitted to the City of Oshkosh for final approval • Within 10 weeks of authorization the Final Plat, (mylar), will be submitted to the Department of Administration, (DOA). The DOA has a 30 day approval timeframe. NOTE: The Winnebago County Planning & Committee has objecting authority for plats within the City of Oshkosh. The DOA will make the required submittal to all objecting authorities. • Within 1 week of receiving the Final plat from the DOA - Plat circulated for signatures • Within 1 week after receiving the plat signed by ail parties — Piat recorded. Revised Budqet The project was authorized with a budget of $362,540 for services as outlined in our original proposal dated October 14, 2014. The total budget was divided into $205,180 for private and $157,360 for public improvements. Amendment 1 services were distributed between both public and private budgets as necessary. The requested proposed additional services for Amendment 2 can be added to the existing public budget. If the addition of the services results in exceedance of the approved budgets due to increased effort hours by the noted project team, a contract amendment request detailing the level of effort and costs will be prepared and submitted to the City for consideration. No increase in budget is requested for these services at this time. The services provided will be performed in accordance with the 2015 Negotiated Fee Schedule and conditions of the General Conditions of Service — City of Oshkosh, April 2009 as provided in our original proposal number OPP-237433, dated February 7, 2014, which are expressly incorporated into, and are an integral part of, our agreement for professional services. AECOM can begin providing these services immediately upon receipt of the signed Amendment Authorization Form (attached).
