The proposed services represent the warranty inspection of the interior of the detention basin prior to completion of the Contractors warranty period on October 22, 2015. The purpose ot the eva{uation is to identify potential observable deterioration of the interior of the structure so as to have corrective action completed by the Contractor prior to conclusion of the warranty period. The following discusses the scope of services to be completed, schedule, information/assistance required from client, and compensation. Scope of Work Services to be provided for the proposed project are as follows: • Task A— Review the 2013 Post Construction Tank Evaluation Memorandum completed by the Project Structural Engineer, dated August 27, 2013 and update the confined space entry Safety Plan and Task Hazard Analysis. • Task B— Complete the confined space entry into the tank by an AECOM Structural E�gineer to review the existing conditions and record changes in conditions previously observed. Record measurements of existing crack monitoring gauges and compare to data obtained '+n 2013. • Task C— Prepare a 2015 Post Construction Tank Evaluation Memorandum documenting conditions observed, comparison of 2013 to 2015 crack monitoring gauge data, and summarize recommendations for corrective actions or additional investigative measures, if needed. Task A: Preparation / Review 2013 Memorandum / Safety Plan Update $1,500 Task B: Confined Space Entry (Structural Engineer) / Tank Evaluation $3,000 Tack C: Report/ 2015 Project Memorandum Terms and Conditions $1,500 TOTAL $6,000 Services described in this proposal will be performed on a time-and-expense/unit-cost basis. The project services will be pertormed in accordance with the 2015 Negotiated Fee Schedule and the April 2009 Negotiated Terms and Conditions between AECOM and the City of Oshkosh, which are expressly incorporated into, and are an integral part of, our contract for professional services. Please indicate your acceptance of this proposal by having an authorized individual of the City execute the attached Authorization Form and return it to our Oshkosh office.
