Director of Public Works City of Oshkosh 215 Church Avenue Oshkosh, wl 54901 By letter dated February 28, 1984, from Wisconsin Department Of Industry, Lat�or & Human Relations to HNTB, DILHR advised that certain plumbing modifications were required for the subject project. The required modifications were detailed on a set of "marked up" plans forwarded witlr the Februa ry 2$th letter. These modifications are listed in the memorandum dated May 1&, 1984 from FINT6 to Fluor Bros. Construction Co. Inc. By letter dated October l, 1984, from Fluor Construction Co. to the City of Oslikosh, Fluor Construction Co., proposed to furnish and install the required plumbing modifications at an additional cost of $5,761.00. NNTB did confirm that the required plumbing modifications were necessary and fiNTB has reviewed Fluor's proposal and recommends that the City of Oshkosh accept the proposal in the submitted amount of $5,761.00. Please find attached four copies of proposed Change Order No. 4 for final approval and processing oy the City of Oshkosh. Please return one signed copy each to Fluor Construction Co. and HNTB.
