SUPPLEMENTAL TASK ORDER N0. 7 TO T[-IE AGRET�..',�IENT DATED 6/20/79 BETWEEN 'I'�iE CITY OF OSHKOSH IV THE STATE OF WISCONSIN AND HOWARD NEEDLES TAM:�F.N & BERGENDOE'F FOR CONSULTANT SERVICES IN COVNECTION WITH IMPROVF.MEV?5 TO THE SFWERAGE SYSTE;1 - 18a - Resolution ��14 Resolution ��14 �-'I.0 Purpose The purpose of this Supplemental Task Order is to est.sblish scopa, schedule and payment provisions for engineering services during execution of the s=werage system facilities construction contrscts as moro specifically described herein. 7-1.1 Co�sultant shall not co�mence work on azy task defined by this Supplemental Tssk Order agreement until autnorize3 by the City. 7-1.2 Agrzemsnt by the City ta the provisions of this Supplementsl Task Order does not obligat° the City to the Consultant for funding or authorizstion of any portion o£ this work until such time that the City authorizes the work defined herein. 7-1.3 The Consultant sha11 not retain tne s°rvices o£ any subconsultant for assistance with the tssk_s described herein vrithout authorization by th= City of Oshkosh. 7-2.0 Scope The scope of the work to be�rformed under the Supplemental Task Order shall be in accordance with th� gzneral provisions described in the Agreement betw�en the City of Oshkosh �nd Howard Needles Tsmmen & Eergendoff, dated June 20, 1979• Thes^_ services are fur- ther defined i� Articles 2.1.¢ Construction Phase, 3nd 2.2 Addi- tional Services, of the Agre�ment, and mor= specifically described as follows. 7-2.1 The CONSULTANT will provide the following engineering services under this Supplemental Task Order: 7-2.1.1 PROJECT MANAGEMENT - Consultant shsll provide overall man- agem=nt servic�s to assist the City with tha administration of project funding programs, administration o£ the project Phsse IA ronstruction Contracts 85-01, 85-02, 85-�3 3n3 85-0�, and oversll mansgement o£ the below listad t3sks. 7-2.�.2 RESIDEIVT ENGINEERING SERVICES - Provide residant en�inser- ing serv;ces d,,.•.;_,•� <ve�„±_o,. ef th� ..,,�:; _�.clu3_d �__ Cc,.- struction Contrscts 85-01 Sewer Intercentor, 85-02�Forca Msin, 85-03 Broad Street Pump Ststion, and 85-07 South Ksin Street Pump Station. 7-2.1.3 SHOP DRAWING SUBMITTAL RPVIEW - Frovi3e reviaw oi shop drawir�gs and contract submittals for conformance with the project plans snd spacifications. 7-2.1.4 CONSTRUCTION STAKEOUT - Provida survey controi and initial stskeout for each o£ the contracts, assist in verifying grades and locations during progress of the construction, assist in finsl inspactio❑ daterminations of horizontal and :� Resolution �k 14 v°rtical alignment of the fscilities, and det�rmine loca- tion ties from new manhol� structures to existing property 1ZOL15 . 7-2.1.5 OPERATION AND MAINTENANCE MANUAL - Pr2par= Operations and Maintenance Manu3ls in accor3ance with the approved Preliminary Plan of Operation and the requirements of the Depsrtment of Natural Resources £or Contracts 85-01, 85-02, 85-03, and 35-07. 7-2.1.6 SOILS TESTING - Provide testing of m3terials offered by the Contractor for compliance with pro,ject backfill material specifications, and fi=1d tssting of Contr3ctor's backfill compaction efforts to verify complianc° with project specifications. 7-2.1.7 RECORD DRAWIVGS - Prepara rocord drawings, based on infor- mation to be furnished by th� constructing Contractor for Contracts 85-01, 85-02, 85-03, and 85-0%. The record drawin�s ,�ill be altered versions of the original project design drawings. Also, the Consultant will provi3= the City with the necessary record dats to compl�tz manhol> dats record cards. 7-3.0 Schedule The work imder this Supplemental Task Order shall commence upon receipt by the Consultant of an executed copy of the Suppl=�ental Task Order, an3 authoria3tion to procee3 from the City to the Con- sultant. Th= field s2rvices to be provided herein will conclu3e upon final acceptance of the work included in Construction Contracts 85-01, 85-02, 85-03, and 85-07. The submittal schedule of the Op2r- stions and M,sintenance Manuals to th= Depsrtment of Nacursl Re- sources for their revi°w snd approval will be in accordance with fuading agency roquirements. Preparation of Record Draxings will be completed in s tim�ly manner after final acceptanc= of the construction o£ work ir.cluded in Contracts E35-01, 85-02, 85-03, and 65-07. 7-4.0 Minority and Small Busin=ss Enterprises (MSBE) Minority or Small Business Enterprises will be retained by the Consultsnt to assist in providing the services included in thzs Supplemental Task Or3=r in accordance with the City of Oshkosh's st3t=_d guideline for minority or s�nall business enterpris= psrtici- pation o£ 10o for work funded wholly or in part by th� Wisconsin Fund grant program. However, no Minority or Sm�ll Business Etiter- prisa will be retained by the Consultant without th� authorization of the City of Oshkosh. 7-5.0 Payment 7-5.1 Payment for the work defined under t:iis Supplementsl Task Order shall be on the basis of a cost plus fixs3 fea in accordance with - 18c - � Resolution �� 14 ths g=neral provisions described in Article 8.1.9 of the Consultant Services Agreement between the City and Consultant dsted Jun= 20, 1979. The total ch.srges for this Supplemental Task Order will not exc=ed $351,533, unless agreed to in writing. 7-5.2 The estimated direct labor costs, indirect costs, other direct costs, and profit hill be itemized in the Supplemental Task Order COST OR PRICE SUMMARY FORf4AT FOR SUBAGREE[4BNTS UNDER WISCONSIN FUND GRANTS form, and on the attachments incidentsl to those forms.
