BE IT FURTHER RESOLVED that money for this purpose is hereby appropriated from Account No. 349-145 - Sanitary Sewer Construction - Construction in Progress. - 39 - SUBMITTED BY APPROVEB � , � RESOLUTION # 18 SUPPLEMENTAL TASK ORDER N0. 12 TO THE AGREEMENT DATED 6/20/79 BETWEEN THE CITY OF OSHKOSH IN THE STATE OF WISCONSIN AND HOWARD NEEDLES TAMMEN & BERGENDOFF FOR CONSULTANT SERVICES IN CONNECTION WITH IMPROVEMENTS TO THE SEWERAGE SYSTEM 12-1.0 Purpose The purpose of this Supplemental Task Order is to establish scope, schedule and payment provisions for Step 1, the facilitfes planning phase of the program as more specifically described herein. 12-1.1 Consultant shall not commence work on any task defined by this Task Order agreement until authorized by the City, 12-1.2 Agreement by the City to the provisions of this Task Order does not obligate the City to the Consultant for funding or authorization of any portion of this work until such time that the City authorizes the work defined herein. 12-1.3 The Consultant shall not retain the services of any Subcon— sultant for assistance with the tasks described herein without authorization by the City of Oshkosh. 12-2.0 Scope The scope of the work to be performed under this Supplemental Task Order shall be in accordance with the general provisions described in the Agreement between the City of Oshkosh and Howard Needles Tammen & Sergendoff, dated June 20, 1979, Article 2.1.2 and 2.1.2.1, "Facilities planning phase. Upon receipt of the City's written authorization to proceed with the facilities planning phase the Consultant shall: "Prepare a facilities planning study and report in ac— cordance with the EPA Step 1 regulations, and as further defined in supplemental task orders." 12-2.1 The CONSULTANT will provide the following engineering services under this Supplemental Task Order for the Phase 2 portion of the program service area defined as District AK, HNTB No. 0140 (CONPRO-5/13) — 39a — —1— RESOLUTION � 18 comprising the area bordered by South Park Avenue, Knapp Street, Tenth Avenue, and Ohio Street: 12-2.1.1 PROJECT MANAGEMENT — Consultant shall provide overall management services to assist the City uith the admioistration of Step 1 activities in this limited portion of the Phase 2 program area, and overall management of the below listed tasks. 12-2.1.2 FLOW MONITORING — Consultant shall monitor uaste— water flows in District AK to provide a flow pattern data base for evaluation of the quantity and likely sources of clearwater infiltration and inflow through a minimum of one wet weather event. 12-2.1.3 CLEARWATER INVESTIGATION — Consultant shall perform visual inspections, smoke testing, and dyed water tracing activities in District AK to document identifiable sources of clearwater infiltration or inflow. 12-2.1.4 FACILITIES PLAN AMENDMENT — Consultant shall prepare an amendment to the program facilities plan, based on results of the above activities, that shall include development of alternative solutions in accordance with the established program criteria for elimination of wet weather surcharging in District AK, cost—effectiveness analysis of the alternative solutions, and sel— ection of a plan for implementation of clearwater source elimination activities, or implementation of Step 2 activities for Interceptor Segment 23, as defined in the approved program facilities plan. Consultant shall prepare amendments to the following facility plan sections to specifically address District AK: Chapter 3— Description of Existing Situation Description of the Existing Sewerage System Chapter 5 — Alternatives Purpose and Scope Alternative M alysis Chapter 6 — Plan Selection Purpose and Scope Cost—Effectiveness Analysis Summary Non—Monetary Impacts Summary Description of the Proposed System Improvements HNTB No. 0140 (CONPRO-5/13) — 39b — —Z � RESOLUTION # 18 Preliminary Cost Estimates Ehvironmental Impact of Selected Plan Public Participation Chapter 7- Arrangements for Implementation Purpose and Scope Implementation Steps Schedule Financial Arrangements Operation and Maintenance Requirements It is understood that the unlisted sections oY the facility plan apply as approved to the limited scope of this continuing Step 1 activity. 12-2.1.5 FACILITY PLAN AMENDMENT SUBMITTAL - Consultant shall work with the City to submit the proposed facility plan amendment to regulatory agency review staff, and to respond to regulatory agency reviex comments as necessary to gain agency approval. 12-3.0 Schedule The rrork under this Supplemental Task Order shall commence upon receipt by the Consultant of an executed copy of the Supplemental Task Order, and written authorization to proceed Yrom the City to the Consultant. Proposed amendments to the facility plan xill be submitted to the City for their reviex prior to submittal to the regulatory agency by no later than June 16, 1986. The services to be provided herein xill conclude upon the regulatory agency's notice of approval of submitted amendments to the facility plan. 12-4.0 Small or Minority Business Ehterprises (MBE) Small or minority business enterprise involvement in this limited scope task order is not necessary. '12-5.0 Pa}rment 12-5•1 Payment £or the xork defined under this Supplemental Task Order, and under all subsequent Subtask Orders, shall be on the basis of a cost plua fiaed fee in accordance xith the general provisions described in Article 8.1.1 of the Con- sultant Services Agreement betxeen the City and Consultant dated June 20, 1979. Total charges for this Supplemental Task Order will not exceed $15,590, unless agreed to in writing. HNTB No. 0140 (CONPRO-5/13) - 39c - -3- 0 RESOLUTION # 18 12-5.2 The estimated direct labor costs, indirect costs, other direct costs, and profit will be itemized in the Supple- mental Task Order COST OR PRICE SUMMARY FORMAT FOR SUB- AGREEMENTS UNDER WISCONSIN FUND GRANTS form, and on the attachments incidental to those forms. IN WITNESS WHEREOF, the parties have caused this Supplemental Task Order No. 12 to be executed the year and the day below written by their proper officers and representatives.
