HomeMy WebLinkAbout07. 15-283 JUNE 9, 2015 15-283 RESOLUTION (CARRIED__7-0______LOST________LAID OVER________WITHDRAWN________) PURPOSE: APPROVE AMENDMENT TO RESOLUTION 15-136 HEART MONITOR MAINTENANCE AGREEMENT 2015-2017 FOR FIRE DEPARTMENT TO PHYSIO-CONTROL INC ($44,290.92) INITIATED BY: PURCHASING DEPARTMENT WHEREAS, the Fire Department requires maintenance agreements for the various Physio-Control, Inc. heart monitor/defibrillator units used by their staff; and WHEREAS, Physio-Control, Inc. is the sole source provider to maintain these units; and WHEREAS, the Common Council previously approved Resolution 15-136 authorizing a three year maintenance agreement for the heart monitor/defibrillators with Physio-Control, Inc., and WHEREAS, it was determined that an error had been made in the pricing of a portion of the plan as identified in the previous resolution and it is therefore necessary to amend Resolution 15-136 to reflect the correct plan and pricing for the heart monitor/defibrillator maintenance agreement. NOW, THEREFORE, BE IT RESOLVED by the Common Council of the City of Oshkosh that Resolution 15-136 is hereby amended to reflect an increase in the cost for maintenance of the Lifepak 15 units from $7000 annually to $10,133.64 annually. BE IT FURTHER RESOLVED that the proper City officials are hereby authorized and directed pursuant to Section 12-11 of the Oshkosh Municipal Code to purchase maintenance agreements from the following approved source: PHYSIO-CONTROL INC. 11811 Willows Road NE PO Box 97006 Redmond, WA 98073-9706 Annual Total: $44,290.92 BE IT FURTHER RESOLVED that the proper City officials are hereby authorized and directed to enter into an appropriate agreement for the purpose of same. ANALYSIS Since the award s[aff has de[ermined that the maintenance wsts for the LifePak 15 units should have been $1Q133.64 annually ($30,400.92 over three years), not the $7,000 annually ($21,000 over[hree years) as sta[ed in [he agreemen[approved in resolution 15-135. The $7,000 cos[ was a clerical ermr[aken from a support agreemen[ we did no[ choose. This change will resul[ in an annual increase of$3,133.64 a�nually overthe three years for a totxl of$9,400.92. As such, staff recommends we amend [his award accordingly. The other main[enance costs from this agreement are cortect. FISCAL IMPACT With this correction the annual Physio-Control Inc. maintenance cost is $4,630 for the Lifepak 1000 uni[s and $10,000 for the Lifepak l5 uni[s. The rotal fiscal impact for [he [hree yeer main[enance agreemen[ (20154017) will be $44,290.92. Finance has confirmed [here are adequa[e funds in the �ire DepartmenPs annual operating budget to cover [his overage (A/NN: 100-0230-6401-00000).
