I. Pro�ect Description Design of a structure for a new stream crossing over Sawyer Creek on the extension of Westhaven Drive. II. Services to be Provided This Proposal is for the specific engineering services listed below. OMNNI Engineers will provide the following professional services for the design and preparation of construction plans for this project. 1. Field survey necessary for the complete structure design including: a. Establish horizontal control from Abbey Drive to CTH E. b. Obtain existing ground profile at the centerline of the proposed road from Abbey Drive to CTH E. c. Estab�ish existing cress-sActions at 104 foot stations, 500 feet on each side of the proposed structure. d. Establish existing stream cross section as required for the hydraulic analysis. e. Locate pertinent topographic features in the vicinity of the proposed structure. 2 . Preparation of the mapping necessary to accomplish the design, based on the field survey. 3 . Coordination with the Wisconsin Department of Natural Re- sources and the US Army Corps of Engineers, and the prepara- tion and submittal of permit applications as required. - 70a - 303 S. OMNNI Engineers will receive proposals for the soils work from qualified consult- ants and the City will be billed for the cost of the soils work at OMNNI ' s cost. 5. Coordinate with the State Preservation officer at the State Historical Society of Wisconsin to determine the impact of this project upon historical or archaeological sites of local , State or National interest. If an Archaeological Survey is required, OMNNI Engineers would receive proposals from qualified consultants and the City would be billed for the cost of the survey at OMNNI ' s cost. 5. rreparati�n oF Construction plans (prepared on DOT Standard Sheets) . a. Title sheet showing project location. b. Typical section sheet. c. Plan and profile sheet showing utilities , proposed right-of-way, property lines , benchmarks and other information on existing features pertinent to the project. The plan and profile sheet will have a horizontal scale 1"=20 ' and a vertical scale of 1"=5 , ' and will show proposed roadway location and vertical alignment for maximum distance of 200 ' each side of the structure. d. Cross sections will be plotted on both approaches, a maximum of 200 ' on each side of the structure, for use in the design of the project, and will become part of the plan. e. Structure plans and details. 7. Preparation of the necessary sp�cifications and estimates to accommodate construction of the structure. 8 . Preparation of the necessary documents for bidding the project. 9 . Other services and considerations a. Attend informational meetings and public hearings as requested by the Municipality. b. Pro�ect Timetable We would anticipate commencing with the work as soon as author- ized to proceed. Upon authorization to proceed, the schedule for completing work shall be as follows: 1. Complete field survey - i month 2 . Submit preliminary plans - 3 months 3 . Submit fina� plans, specifications and estimates - 4 months 4 . Letting - 5 months IV. Fees to be Charqed Fees for the design of the structure and preparation of the bidding documents are based on time and materials not to exceed $13 , 000. 00, with a fixed fee of $1, 950. 00, for a contract total of $14, 950. 00. Fees for the archaeological survey, soil borings, laboratory tests and reports will be charged to the City at OMNNI ' s cost. V. Additional Work If required by the City, additional engineering services for this project will be provided by OMNNI Engineers . Any such additional work will be completed on the basis of base salary times a multiplier of 2 . 4508 , plus reimbursable expenses . Additional services are not anticipated at this time. VI. Citv Responsibility 1. Approval of plans. 2 . Logistics , notices and meeting place for informational meeting if required. 3 . Search of title. 4 . Obtaining easements or rights to lands affected by any increase in the ?00 year flood elevation caused by the new structure. 5. Reproduction and distribution of plans and specs used in bidding the project. 6. Provide OMNNI Engineers with mylar of the contour map of the site. 7 . Provide OMNNI Engineers with right-of-way information for the extension of Westhaven Drive between Abbey Drive and CTH nEu � 8 . Provide OMNNI Engineers with proposed pavement design and typical section for the extension of Westhaven Drive between Abbey Drive and CTH "E" . VIII. Limitation of Liabilitv Westhaven Drive Bridge Engineering Proposal Page 3 of 4 - 7 0 c - ' • Res. #90-36 The Owner agrees to limit the Design Professional 's Liability to the Owner and to all construction contractors and subcontractors on the project, due to the Design Professional ' s professional negligent acts , errors or omissions , such that the total aggregate liability of each Design Professional to all those named shall not exceed $50 , 000 or the Design Professional ' s total fee for services rendered on this project, whichever is greater. IX.
