than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit.
Birds and squirrels are getting into the house through several openings in the fascia and soffit.

Description All rainwater shall be so drained from the roof/ground so as not to allow dampness inside the dwelling unit and so drained as
not to allow standing water,water damage or other detrimental conditions in the dwelling. Roof leaks into kitchen.
Roof repairs are required to be made immediately.

windows,of similar openings 4)Where attached to building surfaces. Discontinue use of extension cord as source of power
for the garage.

Description All dwelling units,vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements)are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants concerning smoke detectors.

Description Two bedrooms were added in the basement by the tenant. These bedrooms were built without the needed building permits
and do not comply with habitability requirements for ceiling height, light and ventilation. The rooms may only be used as
storage areas. Discontinue the use as bedrooms immediately.

Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props. :
Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather
tight. Front window is in poor condition and shall be repiared or replaced.

same free of conditions reflective of deterioration and/or inadequate maintenance. Areas where paint is chipping/peeling shall
be repaired by scraping and painting or covering with a weathe,r resistant material.

Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Numerous electrical violaitons were found at inspection. See
photos for locations. A licensed electrician shall correct the violations. Tenant stated that he installed the wiring for the
basement.

Description Installation of electrical wiring in rental dwellings requires a licensed electrician and an electrical permit. No permit on file for
this work.

Description Numerous violations of the State Plumbing Code were found at inspection. See photos for locations. A licensed plumber with
permit shali make the necessary corrections.

Description No person shall perform or permit the performance of any plumbing work without properly obtaining a permit. A licensed
plumber is required for all work to be done in rental units.
No permit on file for replacing a water heater or repiping.

�Description No owner or operator shall rent to any other person for occupancy or allow any person to occupy any dwelling unit or premises
unless it is safe and clean and complies with all provisions of the Minimum Housing Code and Property Maintenance Code.
Summarv Contact me on or before 10/15/12 to arrange for a re-inspection of your property to determine the status of the repairs. Should
you fail to contact me on or before 10/15/12 citations will be issued to you in the amount of$232.00 per day of violation, per
violation.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 9/18/2009
