Box 1130 Oshkosh, Wisconsin 54903-1130 Subject: Agreement for Structural Engineering Design and Construction Related Phase Services,CY2015 City of Oshkosh Department of Public Works Capital Improvement Projects—AECOM Proposal No. OPP-323242 Dear Mr. Rabe, AECOM Technical Services, Inc. (AECOM) is pleased to provide this Agreement for Structural Engineering Design and Construction Related Phase Services, for the City of Oshkosh Department of Public Works Capital Improvement Projects for the calendar year 2015(CY2015). The CY2015 projects include, but are not limited to, street and public utility improvement, storm water watershed design/improvement, and facility construction/improvement. The scope of work includes, but is not limited to: • Preparation of Design Drawings and Review of Technical Specifications(structural engineering) • Assistance during Construction Phase of the Projects Budget and Schedule This Agreement is intended for use as a general account agreement for engineering support services to the Department of Public Works, under which services will be authorized and invoiced on a 'work order' basis. For each project/work order request for engineering support,AECOM will complete a Project Authorization Form, or"Work Order" (attached) containing the scope of work and estimated costs, and submit for approval to appropriate personnel in the Department of Public Works. Upon receipt of the authorization and issuance of a City of Oshkosh Purchase Order, AECOM will set up a task under this authorized AECOM agreement. This AECOM agreement will be considered complete when all the work and deliverables for the authorized work orders/Purchase Orders in CY2015 has been completed and submitted. The services described in this proposal will be provided on a time and materials basis. Based upon the above scope of services,the estimated budget for this agreement to provide engineering design and construction related phase services for City of Oshkosh CY2015 Department of Public Works capital improvements projects is$50,000. The services provided will be perFormed in accordance with the 2015 Negotiated Fee Schedule and conditions of the General Conditions of Service—City of Oshkosh,April 2009(attached), which are expressly incorporated into, and are an integral part of, our agreement for professional services. AECOM can begin providing these services immediately upon receipt of the signed Authorization Form (attached).
