Kempinger, Supervising Architect Building, heating, ventilation and alteration plans have been reviewed by the Department of Industry, Labor and Human Relations for compliance with the important code requirements. The plans are stamped "Conditionally Approved," and construction may proceed subject to local regulations, but all items that are required to be changed by this letter must be corrected before commencing that part of the work. You are hereby advised that the owner as defined in Chapter 101.01 (2) (i) of the Wisconsin State Statutes is responsible for all code requirements not specifically cited herein. Code requirements are set forth in Chapters 50 thru 64 of the Rules of the Department. The building will be inspected pected during and after construction by a deputy of this Department to insure complete eemplianee with Wisconsin Codes. The owner may notify the state building inspector listed below if a final inspection is desired before taking possession of the building. Ind 50.15 Evidence of approval. The architect, professional engineer, designer, builder or owner shall keep at the building, one set of plans bearing the stamp of approval. This approval covers the alterations as shown on the plans. The alterations are for the first floor northwest corner of the building. Yarbro-Kempinger, Architects, Inc. September 9, 1982 Page 2 Ind 51 .07 The waiting room 105, vestibule 106 and corridor 114 and corridor 113 are required to have flame spread rating of 75 or less. The materials used on the walls and ceiling in these areas shall have a flame spread of 75 or less. The vinyl and the cek which you have provided does not provide the less than 75 flame spread. Ind 64.41 You are using the plenum as a return air plenum, therefore, there shall be DO carbustible materials in the plenum. You have shown numerous Places where the ?. .x 4 studs are in the plenum area. These are not permitted. Ind 51 .03, Table A This building is required to be of fire-resistive construction, therefore , all of the wood studs which you have shown on the project shall be removed and replaced with metal studs. GENERAL CONSTRUCTION WORK Alterations to City Hall For Probation and Parole 215 Church Avenue City of Oshkosh, Wisconsin ARTICLE 3 TIME OF COMMENCEMENT AND SUBSTANTIAL COMPLETION The Work to be performed under this Contract shall be commenced 7 September 1982 and, subject to authorized adjustments, Substantial Completion shall be achieved not later than 1 November 1982 (Here insert any special provisions for liquidated damages relating to failure to complete on time.) If substantial completion is not achieved on or before 1 November 1982 , liquidated damages in the amount of $25. 00 will be assessed for each consecutive calendar day required by the Contractor to achieve substantial completion. There will be no retainage on balance to completion. Upon determination by the Architect/Engineer that satisfactory progress has been made, payments authorized at time of substantial completion shall be for total retainage in contract except that an amount equal to twice the estimated cost to complete or correct items on a tentative list of uncompleted items shall be retained until final completion. Payments due and unpaid under the Contract Documents shall bear interest from the date payment is due at the rate entered below, or in the absence thereof, at the legal rate prevailing at the place of the Project.
