AMENDMENT #1
TO
AGREEMENT FOR GRANT ADMINISTRATION SERVICES BETWEEN THE EAST
CENTRAL WISCONSIN REGIONAL PLANNING COMMISSION AND'THE CITY OF
OSHKOSH, WINNEBAGO COUNTY, WISCONSIN FOR GRANT ADMINISTRATIVE
SERVICES FOR A COMMUNITY DEVELOPMENT BLOCK GRANT- EMERGENCY
ASSISTANCE PROGRAM (CDBG — EAP)
This Contract, made and entered into by and between the City of Oshkosh and the East Central
Wisconsin Regional Planning Commission, is hereby amended as follows:
1. Extend Contract Performance Period from September 30, 2012 to December 31, 2013.
In order to accomplish the above, the original contract entered into on October 29, 2010, is
amended as follows:
Item V. Time of Performance, will be changed to read as follows:
This contract will be in effect as of the date first above written through December 31, 2013 and
may be extended or shortened upon the mutual agreement of the Commission and the City,
based on unforeseen construction delays/accelerations, or as need to fully administer the grant
in a timely manner. This extension is necessary to allow time to properly complete construction
of the James Road Stormwater Detention Basin.
In APPENDIX B, TIMETABLE modify the Timetable starting with September 30, 2012 as follows
ON OF BEFORE ACCOMPLISHMENTS
September 30, 2012 Complete Environmental Review.
Report date of bid advertising and bid opening.
Submit project Notice of Contract Award an project start date.
Submit project schedule, bid summary, and project costs.
Submit Beneficiaries Data and Accomplishments Reports.
December 31, 2012 Submit Beneficiaries data and Accomplishments Reports.
Arrange for audit, if necessary.
January 15, 2013 Submit audit-scheduling letter to Department of Administration.
March 19, 2013 Submit Semi-Annual Labor Standards Enforcement Report.
March 31, 2013 Submit Beneficiaries Data and Accomplishment Reports.
EC Contract Number 2382
June 30, 2013 Submit Beneficiaries Data and Accomplishment Reports.
September 19, 2013 Submit Semi-Annual Labor Standards Enforcement Report.
December 31, 2013 Complete closeout report, Beneficiaries Data and
Accomplishments Report, and Final Labor Standards Compliance
Report and submit to the Department of Administration.
All other requirements of the contract will remain as stated.
