Introduction Having had no further contact or confirmation of compliance concerning this order,an inspection was made on 12/5/13 to
determine the status of the corrections. The foilowing uncorrected/reoccurring violations were noted:

Description No owner,agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard.This includes the proper storage&removal of garbage, debris and yard waste.
*"All of the boxes, garbage, plastic items,wood,furniture and all debris that is around the dumpsters and scattered around the
property must be cleaned up and disposed of by the compliance date. Please have the overflowing dumpsters emptied.""
Summarv A service fee of$50.00 will be charged to you for failure to comply with this notice.The service fee will be billed to you
separately. Should you fail to pay the bill it will be added to your taxes as a special assessment. The City of Oshkosh
Sanitation department has been notified that your property is to be cleaned up since you did not respond to this notice or the
Abatement Letter that was sent to you. All costs associated with the cleanup will be billed to you as well.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 11/28/2013
Office hours are Monday through Friday 7:30 a.m. -4:30 p.m.or by appointment.To schedule inspections please call the Inspection
Request line at 236-5128 noting dress, permit number(when applicable),and the nature of what needs to be inspected.

violations were abated by the City. You will be billed accordingly for the services that were provided under separate billing.

City of Oshkosh. Failure to pay the assessed service fee will result in fee being added as a special assessment on your
property tax bill.
