Introduction Having had no further contact or confirmation of compliance conceming this order, an inspection was made on Nov 4, 2013 to '
determine the status of the corrections. While it was noted that the section of concrete walkway had been turned over, it
remains in violation of the municipal code mentioned below.

Description No owner, agent or occupant shall allow on any residential premises any junk,debris or other condition,which creates a public
nuisance, eyesore and/or hazard.This includes the proper storage&removal of garbage, debris and yard waste.

Summarv A service fee of$50.00 will be charged as a result of this notice. It is the responsibility of every property owner to confirm
compliance with these orders before the compliance date(s)as specified. Please contact this office on or before the scheduled
due date(s)to schedule a re-inspection of the property. If you do not contact this office to verify compliance your property will
be re-inspected after Nov 11,2013. If it is found that you are still not in compliance you will be issued a service fee of$50.00
and the violation will still be required to be corrected.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and retum it to the Inspection Services Division by the Compliance Date of 11/11/2013 '

Introduction An inspection of your property during inspections in the neighborhood revealed the following violations of the Oshkosh
