Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. if you have any questions

Item# 1 Code 16-35A(3) Complies No Comply By 09/12/2012 IMMEDIATELY
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Toilet is leaking through the base.
Item# 2 Code 16-33A(8) Complies No Comply By 09/12/2012 IMMEDIATELY
Description When provided or used in any dwelling unit subject to the provisions of this Article, natural gas ranges, ovens and oven/range
combinations of any type shali be installed, maintained and used or operated in a safe and non-hazardous manner. One of the
burners is not working on the stove.
Item# 3 Code 16-37A(5) Complies No Comply By 09/12/2012 IMMEDIATELY
Description Every owner shall be responsible for extermination of any insects, rodents or other pests whenever infestation occurs in more
than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit. Tenant
stated that they have had problems with bats in the storage area. Several holes found where bats couid enter the dewlling unit.

Description Stairways in one and two family dwellings shall be in sound condition, equipped with proper handrails&guardrails installed in
accordance with COMM 21.04 , safe to use and shall be equipped with treads and risers reasonably uniform. Secure or
remove the newel post top.

Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage&removal of garbage, debris and yard waste. Tenant was
informed to remove the washer and dryer from the yard.

Description All living units shall have immediate access to electrical service panels without passing through another living unit or
occupancy. Upper tenant shall be given access to the basement for access to their electrical panel.
Item# 7 _ Code 17-36A Complies No Comply By 10/05/2012
Description The exterior of every structure or accessory structure including fences shall be maintained by the owner, occupant or person
authorized to use same free of conditions reflective of deterioration and/or inadequate maintenance. Repair damaged siding
areas. Areas where paint is chipping/peeling shall be repaired by scraping and painting or covering with a weather resistant
material. The shed does not have a finished exterior material (osb is not intended to be used as a finished exterior material).
Replace the broken basement windows.

Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Replace the flooring in the bathroom. If the subfloor is water damaged it may also have to be replaced. Repair the shelf in the
kitchen. Replace the side entrance door(threshold,frame and door in poor condition).

Description All habitable room windows/door windows shall be effectively screened with screens in good serviceable condition. Install
screens where missing.
Item# 10 Code 16-35A(1)(c) Complies No Comply By 10/05/2012
Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props.
Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather
tight. Check all windows to ensure they will remain in the open position without the use of props. Windows were propped at
inspection.
Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
andlor occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 10/4/2012
