Introduction Having had no further contact or confirmation of compliance concerning this order, an inspection was made on Sept 20,2013
to determine the status of the corrections. The following uncorrected/reoccurring violations were noted:

nuisance, eyesore and/or hazard.This includes the proper storage&removal of garbage,debris and yard waste.
*'All debris, plastic items,wood, bottles and furniture that is scattered around the property and on the porch must be removed
by the compliance date.**
Summarv A service fee of$50.00 will be charged as a result of this notice. It is the responsibility of every property owner to confirm
compliance with these orders before the compliance date(s)as specified. Please contact this office on or before the scheduled
due date(s)to schedule a re-inspection of the property. If you do not contact this office to verify compliance your property will
be re-inspected after Oct 1, 2013. If it is found that you are still not in compliance you will be issued a service fee of$50.00 and
the violation will still be required to be corrected.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the tnspection Services Division by the Compliance Date of 10/1/2013

Introduction An inspection of your property following a complaint revealed the following violations of the Oshkosh Municipal Code,which
shall be corrected by compliance dates specified for each item. If you have any questions regarding the requirements of this

Description No owner,agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard.This includes the proper storage&removal of garbage, debris and yard waste.
**Remove the mattress and box spring,the seats,chairs, plastic items,garbage and all debris from around the house and on
the porch.**
Summarv A service fee of$50.00 will be charged as a result of this notice. It is the responsibility of every property owner to confirm
