Introduction Based on our previous telephone conversations you were granted an extension through Sept 24,2013. Having had no further
contact or confirmation of compliance concerning this order,an inspection was made on September 25,2013 to determine the
status of the corrections. The following uncorrected violations were noted:
Item# 1 Code 17-36A Complies No Comply By 10/14/2013 IMMEDIATELY
Description The exterior of every structure or accessory structure, residential and non-residential, shall be maintained by the owner,
occupant or person authorized to use same, in good repair and all exterior surfaces free from crumbling stone or brick,
excessive chipped, peeling,or lack of paint, missing, broken or deteriorating steps, conditions reflective of deterioration and/or
inadequate maintenance or as may tend to depreciate property values in the area or create a nuisance or hazard

Description All openings where rodents, pigeons, insects might enter shall be properly and effectively sealed.

Item# 3 Code 17-366 Complies No Comply By 10/14/2013 IMMEDIATELY
Description� No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage&removal of garbage, debris&yard waste.

Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Address the violations listed in this correction notice immediately and contact me to confirm compliance by the due
dates. If I do not hear from you by October 14,2013 citations may be issued and the violations will still be required to be
corrected. If you have any questions regarding this notice please contact me immediately at(920)236-5054.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 10/14/2013
