introduction An inspection of your rental property(living unit)following a tenant compiaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 _ Code MUN 16_37A(5) _ Complies No Comply By 01/29/2013 IMMEDIATELY
Description Every owner shall be responsible for extermination of any insects, rodents or other pests whenever infestation occurs in more
than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit. A
significant cockroach infestation was found in the kitchen. Contact an exterminator immediately to begin a treatment plan to
eliminate the problem.
Item# 2___ Code MUN 16-37A(1)_ _ Complies No Comply By 01/29/2013 IMMEDIATELY
Description Every occupant of a dwelling unit shall maintain in a clean, safe&code compliant condition that part of the dwelling unit or
yard he occupies and controls. The tenant was told to begin cleaning the kitchen. Cockroach feces found on walls, cabinets,
countertops and other areas in the kitchen. Stove shall be cleaned of grime and grease. Tenant also told to remove the
garbage and recyclables from the basement.
Item# 3 _ Code MUN 16-34A(3)(a) __ Complies No__ Comply By 01/29/2013 IMMEDIATELY
Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Electrical outlet pulling out of the wall behind the couch in the
living room. Missing cover plate on receptacle in the kitchen. Install light bulbs in any open sockets(basement). Contact an
electrician for repairs to the GFCI receptacle in the kitchen that is not functioning and to properly install teh wiring and outlet for
the dryer in the basemen (or remove). :
Item# 4 Code MUN_16-35A(3) __ _ _ Complies No Comply By 01/29/2013 IMMEDIATELY
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Contact a plumber to make repairs to what is leaking in the 1 st floor
bathroom. Shower and/or toilet is leaking. Leak can be seen down into the basement. Check the kitchen sink for leaks also.
Water damage found in the cabinet. Remove the garden hose from the basement(connected to laundry supply).
Item# 5 Code ss 101.647 Complies No Comply By 01/29/2013 IMMEDIATELY
Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor
level except the attic, garage, or storage area of each dwelling unit. Except as provided under par. (b)the occupant of the
dwelling unit shal�maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that ;
the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice, any
maintenance necessary to make that carbon monoxide detector functional.
Item# 6 _ Code MUN 16_33A(7)_ Complies No Comply By 01/29/2013 IMMEDIATELY
Description All dwelling units,vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements) are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants concerning smoke detectors.
Item# 7 Code MUN 16-35A(1)(c) _ Complies No Comply By 02/20/2013
Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props.
Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather
tight. Replace afl broken panes of glass. Repair the sashes where the weights have been cut off. Windows will not remain in
the open position.

Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair. :
Once the leak is stopped in the bathroom have the carpets cleaned or replace the flooring. Replace the caulking in the
shower. Caulking is peeling away and is discolored. Repair the water damaged wall area behind the upstairs toilet. Repair
the peeling paint on the walls. Repair the flooring that is peeling up in the kitchen. Repair the water damaged kitchen cabinet
or replace. Replace the rusted trim ring in for the light in the shower. Repair/replace damaged flooring by back door.

Description Continuation of item#8-- Replace or remove the back storm door.
Item# 10 _ Code MUN 16-33A(6) _ Complies No Comply By 02/21/2013
Description Stairways in one and two family dweflings shall be in sound condition, equipped with proper handrails&guardrails installed in
accordance with SPS 321.04 , safe to use and shall be equipped with treads and risers reasonably uniform. Install a handrail
for stairs going to the basement.
Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 2/20/2013
