APPLICATION AND AGREEMENT ON
TRAFFIC SIGNAL INSTALLATION
MAINTENANCE AND OPERATION FILED

The City of Oshkosh does hereby make application for permission to
install, operate and maintain a traffic control signal at the intersection
of South Park Avenue and Knapp Street for the purpose of controlling the
flow of traffic.
The following conditions precedent to approval of a signal are
acknowledged and accepted by the City of Oshkosh which further agrees that
it will not deviate from these conditions except upon written agreement
between the Department and the City:
1. The design, installation and operation of the signal will comply
with the regulations of the Department.
2. The cost of installation, maintenance and operation of the signal
will be paid by the City of Oshkosh.
3. No separate interval will be provided for pedestrian movement
during which time no vehicular movement may take place.
4. The number and location of signal faces, the sequence of operation
and the signal timing must be in accordance with Department
regulations.
5. The traffic signal will be of the fully actuated type with
pedestrian push buttons and vehicle detectors on South Park and
Knapp Street. The recall switches shall be kept in the off
position for all phases, except for temporary periods due to
equipment failure.
6. The normal operation of the signals, during which the right-of-way
is successively assigned to the various vehicular and pedestrian
movements, shall be in effect 24 hours.
7. Parking will be prohibited at the following places:
a. For a minimum of 200 feet on all approach roadways
b. For a minimum of 100 feet on all exit roadways
S. The City will notify the District Director of the Department when
the signal installation is complete, and a representative of the
Department shall inspect the installation for compliance with the
conditions contained herein before the signal is placed in
operation by the City.
9. The City, in collaboration and with the approval or at the
request of the Department and at its own expense, will make such
adjustments in the equipment and manner of operation of these
signals as are deemed necessary for public safety and the
facilitation of traffic movement.
10. Pedestrian "Walk" - "Don't Walk" indications will be provided for
on all crosswalks. The "Walk" indication will be activated by
push buttons on all approaches and will terminate sufficiently
before the start of the cross street green interval so that a
suitable clearance time (flashing "Don't Walk") is indicated for
pedestrians.
11. All necessary warning, regulatory and informational signs for
vehicular and pedestrian traffic entering this intersection shall
be provided by the City.
12. This agreement supersedes all former agreements for this
intersection, and the Department's approval of this agreement
shall terminate and the agreement shall be void if the installation
of the signal is not completed within one year following approval
by the Department.

OF A TRAFFIC CONTROL SIGNAL
The installation, maintenance and operation of a traffic control
signal by the City of Oshkosh at the intersection of South Park Avenue and
Knapp Street, is hereby approved, subject to the conditions contained in
the preceding application and agreement.
