DATE: September 19, 2013 RE: Amendment to Agreement for Construction Testing Services for the James Road Area Detention Basin/ Midwest Engineering Services, Inc. (MES) ($16,200) In December 2012, a contract was awarded to MES to provide construction testing services for the James Road Area Detention Basin. During the course of performing those services, it was determined that additional services were necessary due to additional onsite time required while the contractor repaired areas that were not constructed properly initially. These services include additional site time, meetings, construction materials lab testing, and concrete cylinder testing. The amount of this contract amendment is $16,200. This will raise the total contract amount to $39,290 (not to exceed). These services will be funded through the Storm Water Utility. Chapter 12 of the Municipal Code of the City of Oshkosh provides that professional services of a specialized nature, including engineering services, may be procured without the use of formal, sealed quotes. In accordance with Section 12-16 of the Code, I am hereby submitting this report regarding this contract amendment. Pleas advise me if you have any questions concerning this contract amendment.
