Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 16-34A(3)(a) Complies No Comply By 05/17/2012
Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Install cover plates for any open junction boxes. Discontinue
the use of extension cords to provide pernament power for basement lights. Properly abandon electrical wiring (wires found
hanging). Install knockout plugs on open junction box knock outs. Properly support NM cable in basement. Install cover
plates on outlets where missing. Fix hanging fixtures in closets and outlets that pull out or push into the wall. Contact a
licensed electrician.
Item# 2 Code MUN 16-33A(7) Complies No Comply By 04/24/2012 IMMEDIATELY
Description All dwelling units,vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements)are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants concerning smoke detectors. 2nd floor did not have a working smoke detector.
Item# 3 Code MUN 16-35A(3) Complies No Comply By 05/17/2012
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Laundry hook up found leaking. Supply piping is undersized.
Abandon or repipe. See photo for location.
Item# 4 Code MUN 16-35A(1)(a) Complies No Comply By 05/17/2012
Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Replace the water damaged ceiling tiles.
Item# 5 Code MUN 16-34A(2)(c) Complies No Comply By 05/17/2012
Description All habitable room windows/door windows shall be effectively screened with screens in good serviceable condition. Replace
the two torn screens.

Item# 6 Code MUN 17-36D Complies No Comply By 05/17/2012
Description All openings where rodents, pigeons, insects might enter shall be properly and effectively sealed. Repair the holes in the
fascia/soffit.

specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property.
�olations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 5/17/2012
