Introduction Having had no further contact or confirmation of compliance concerning this order, an inspection was made on October 3,
2013 to determine the status of the corrections. The following uncorrected/reoccurring violations were noted:

Description No owner,agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a
public nuisance,eyesore and/or hazard.This includes the proper storage&removal of garbage, debris and yard waste.

Description The parking of vehicles&equipment not intended for residential use on residential premises is prohibited.

Description No building or structure or any part thereof shall be moved, built, enlarged, altered, or demolished within the City uniess a #
� permit is obtained. There is no permit on record for re-roofing, expanding the gravel driveway/parking area,the side patio �
door and interior remodeling at this address. A site plan will be required to be submitted for the expanded parking area
when applying for the building permit. Please contact Todd Muerher in Planning for any questions on the parking
regulations at 236-5057. Permits will also be required for the required light off the patio door and code complaint steps that
are currently missing,
Summarv A service fee of$50.00 will be charged as a result of this notice. It is the responsibility of every property owner to confirm
compliance with these orders before the compliance date(s) as specified. Please contact this office on or before the
scheduled due date(s)to schedule a re-inspection of the property. If you do not contact this office to verify compliance your
property will be re-inspected after Oct 20, 2013. If it is found that you are still not in compliance you will be issued a service
fee of$50.00 and the violation wili still be required to be corrected.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 10/20/2013
Office hours are Monday through Friday 7:30 a.m. -4:30 p.m. or by appointment. To schedule inspections please call the Inspection
Request line at 36-5128 noting the a dress, permit number(when applicable),and the nature of what needs to be inspected.
