Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 16-37A(1) _ Complies No _ _ Comply By 08/21/2012 IMMEDIATELY
Description Every owner of a dwelling shall be responsible for maintaining in a safe, clean and good condition all communal, shared or
public areas of the dwelling and appurtenant premises. Maintenance was on site and was going to clean off the basement
floor.
Item# 2 Code SPS 382.38_ Complies No Comply By 08/21/2012 IMMEDIATELY
Description The allowable discharge point for the condensate from a furnace or water heater is to a municipal �anitary sewer or combined
sanitary-storm sewer. Condensate was discharging to a sump pit.
Item# 3 Code MUN 16-33A(9) Complies No Comply By 08/21/2012 IMMEDIATELY
Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in
Section 16-22 OMC. Install deadbolts on front and back doors for both units at1429 and 1431.
Item# 4 Code MUN 17-366 Complies No Comply By 08/21/2012 IMMEDIATELY
Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage&removal of garbage, debris and yard waste. Remove the
brush pile that is behind the garage.
Item# 5 Code MUN 16-35A(1)(c) Complies No_ _ _ Comply By 09/12/2012
Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props.
Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather
tight. Replace the broken pane of glass in the upstairs bedroom window. Repair the window where the pane is not glazed in
place.

Description The exterior of every structure or accessory structure including fences shall be maintained by the owner, occupant or person
authorized to use same free of conditions reflective of deterioration and/or inadequate maintenance. The roof is in poor
condition and needs to be replaced. Permit required.
Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property. If you
are unable to make the necessary repairs by the listed compliance date(s), please submit a schedule of corrections to be
made, along with competion dates. Submitting a schedule can result in extended periods of time to make repairs on those
items not Life/Safety issues.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 9/9/2012
