If you have any questions regarding the requirements of this notice please contact me immediately at(920)236-5137. , Item# 1 Code MUN 16-33A(9) Complies No Comply By 08/02/2013 Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in Section 16-22 OMC. *"Door knob is stripped out. Replace the knob. Item# 2 Code ss 101.647 Complies No Comply By 08/02/2013 Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor level except the attic, garage, or storage area of each dwelling unit. Except as provided under par. (b)the occupant of the dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice,any maintenance necessary to make that carbon monoxide detector functional. Item# 3 Code 16-34A(3)(a) Complies No _ Comply By 08/25/2013 Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be connected to the source of the electric power in a safe manner. *"The roof had leaks where water damaged the ceilings. Replace the fixtures that were in contact with the leaking water. Item# 4 Code 16-35A(1)(a) Complies No Comply By 08/25/2013 Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair. "*Repair all water damaged surfaces including the ceilings and floors. Install 5/8"drywall on the ceiling where portions of the ceiling fell down. Repair the sagging section of ceiling. Scrape and paint the bathroom walls/ceiling where paint is chipping and peeling. Patch all holes in the walls. Replace or have the carpeting cleaned in the bedroom where the ceiling fell. See photos. 21308 Page 1 of 2 Item# 5 Code MUN 16-33A(7) Complies Yes Comply By 08/02/2013 Description NOTE: At inspection it was found that the smoke detector was disabled (battery disengaged). The battery was put back in and the detector functioned properly. Tenants responsibility to check the detectors monthly and not tamper with them. Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as specified.
