Item# 1 Code ss 101.647 Complies No Comply By 06/11/2013 IMMEDIATELY Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor level except the attic, garage, or storage area of each dwelling unit. Except as provided under par. (b)the occupant of the dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice,any maintenance necessary to make that carbon monoxide detector functional. **At inspection no CO detectors found in basement or 1 st floor. Item# 2 Code 16-33A(9) Complies No Comply By O6/11/2013 IMMEDIATELY Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in Section 16-22 OMC. ""At inspection the front door did not latch. The catch did not extend far enough to engage with the stirke. The door was in overall poor condition and should be replaced. The jamb side is split and held together with several screws-looks as though it was kicked in at one point. If the door will be replaced additional time can be given as long as a temporary repair is made to the locking mechanism. Item# 3 Code 16-35A(3) Complies No Comply By 06/11/2013 IMMEDIATELY Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall be replaced/repaired by a licensed plumber. ""The tenant stated that they have had problems with pipes from the upper unit leaking through their ceiling on a regular basis. Water staining was found on the ceiling grid and panels. Make repairs to any plumbing leaks. Toilet tank leaks and is leaning back. Bath tub spout leaks. Drain for kitchen sink leaks into the cabinet below. Piping for the laundry drain,furnace condensate and sump pump do not comply with plumbing code. Install vacuum breaker on hose bibb. Item# 4 Code 16-34A(3)(a) Complies No Comply By 06/11/2013 IMMEDIATELY Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be connected to the source of the electric power in a safe manner. *"Install light bulbs in all open sockets in the dwelling unit and baement. Contact an electrician to remove the illegal wiring in the bathroom cabinet. Item# 5 Code SPS 321.08(2) _ Complies No Comply By O6/11/2013 IMMEDIATELY Description Dwelling unit separation: Floors and ceilings-A fire protective membrane of one layer of 5/8 inch Type X gypsum wallboard with joints in compliance with Sub(1)(a)(2)shall be proveded on the ceiling beneath the floor construction that provides the separation. **At inspection it was found that portions of the lathe and plaster were removed and a suspended ceiling was installed beneath. The ceiling is required to be drywalled to provide the fire separation from the lower unit to the upper unit. Make immediate arrangements for a contractor to install the drywall-additional time can be given to complete the work. Item# 6 Code 16-33A(6) Complies No Comply By 06/11/2013 IMMEDIATELY Description Stairways in one and two family dwellings shall be in sound condition, equipped with proper handrails&guardrails installed in accordance with SPS 321.04 ,safe to use and shall be equipped with treads and risers reasonably uniform. **Install a handrail for stairs to the basement. Item# 7 Code 16-37A(1) _ Complies No Comply By O6/11/2013 IMMEDIATELY Description Every owner of a dwelling shall be responsible for maintaining in a safe, clean and good condition all communal, shared or public areas of the dwelling and appurtenant premises. *'Tenant stated that items were left in the basement from previous tenants. Remove any items from the basement that have been left behind. 21012 Page 1 of 3 ,Item# , 8 Code SPS 323.14 Complies No ___ _ Comply By 06/11/2013 IMMEDIATELY Description Gas-fired clothes dryers shall be provided with metal venting that terminates outside the structure. **Eliminate the plastic venting and install flexible or rigid metal. Item# 9 Code 1635A(1)(a) Complies No Comply By 07/04/2013 Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair. '*Repair or remove the damaged front storm door. Repair holes in walls and repaint. Replace water damaged ceiling tiles. Install covers for shower head and control. Replace flooring in the bathroom and where damaged in the kitchen. Repair counter tops in the kitchen so the seams are level. Replace broken cabinet door. Carpeting was not seamed together and transition strips were not installed. Item# 10 Code 16-34A(2)(c) Complies No Comply By 07/04/2013 Description All habitable room windows/door windows shall be effectively screened with screens in good serviceable condition. *'Install screens on windows where screens are missing. Item# 11 Code 16-35A(1)(a) Complies No Comply By 07/04/2013 Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props. Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather tight. ""Make repairs to the window where expanding foam has been installed. Item# 12 Code 16-34A(2)(c) Complies No Comply By 07/04/2013 Description Every dwelling unit shall have heating facilities which are capable of safely and adequately heating all habitable rooms and bathrooms to a temperature of at least 67 degrees F with an outside temperature of minus 15 degrees without forcing the heating system. All habitable rooms to be supplied with adequate heat sources. All heating shall be in operation to maintain this minimum during all occupied periods. "`No heat source found for a bedroom on the 1 st floor. Install a heat duct for the room or install baseboard heat. Item# 13 Code 17-36A Complies No Comply By 07/04/2013 Description The exterior of every structure including fences shall be maintained by the owner in good repair. All exterior surfaces thereof be kept painted or covered with exterior siding for purposes of preservation and appearance. All building foundations shall be maintained. '"Mortar in foundation missing and cracking. Tuck point the areas where it is cracked&missing. The shed is in poor condition-rusted roof panels, bent up panels, gaps in the sidewall,etc. Make repiars to the shed or remove it. 21012 Page 2 of 3 .Item# � 14 Code 16-35A(6) Complies Compiy By 07/04/2013 Description No owner or operator shall rent to any other person for occupancy or allow any person to occupy any dwelling unit or premises unless it is safe and clean and complies with all provisions of the Minimum Housing Code and Property Maintenance Code. **The current tenant is moving out. The unit cannot be rented out again until the interior violations are corrected and compliance is verified through a re-inspection of the unit. Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property. If you are unable to make the necessary repairs by the listed compliance date(s), please submit a schedule of corrections to be made, along with competion dates. Submitting a schedule can result in extended periods of time to make repairs on those items not Life/Safety issues. Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment and/or occupancy.
