Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 17-366 Complies No_ Comply By 08/21/2012
Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage&removal of garbage, debris and yard waste. Remove all
junk, debris, garbage, etc. from the yard. Recycling carts had garbage mixed in them and will not be picked up. Separate the
garbage and recyclables and place them out for collection on your collection day.
Item# 2 Code MUN 16-33A(9) Complies No Comply By 09/12/2012
Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in
Section 16-22 OMC. Repair or replace the damaged back door(looked to be kicked in).
Item# 3 Code MUN 16-33A(8) ____ Complies No Comply By 08/21/2012 IMMEDIATELY
Description When provided or used in any dwelling unit subject to the provisions of this Article, natural gas ranges, ovens and oven/range
combinations of any type shall be installed, maintained and used or operated in a safe and non-hazardous manner. Repair the
oven door so that it will stay shut.
Item# 4 Code MUN 16-35A(1)(a) Complies No Comply By 09/12/2012 :
Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Replace the flooring in the bathroom and the kitchen. Cigarette burns, holes and tears found in the floor coverings.

Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Fix the light fixture that is hanging off of the ceiling.

Description The exterior of every structure or accessory structure including fences shall be maintained by the owner, occupant or person }
authorized to use same free of conditions reflective of deterioration and/or inadequate maintenance. The roof is in poor
condition and needs to be replaced. Repair or replace all damaged siding. Replace the missing soffit pans and aluminum
fascia. Replace the broken window.
Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property. If you
are unable to make the necessary repairs by the listed compliance date(s), please submit a schedule of corrections to be
made, along with competion dates. Submitting a schedule can result in extended periods of time to make repairs on those
items not Life/Safety issues.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of g/�2/2012
