Introduction An inspection of your property following a complaint revealed the following violations of the Oshkosh Municipal Code,which

Item# 1 Code 17-36B Complies No Comply By 06/28/2013 IMMEDIATELY
Description No owner,agent or occupant shall allow on any residential premises any junk,debris or other condition,which creates a public
nuisance,eyesore and/or hazard.This includes the proper storage&removal of garbage,debris and yard waste.
"'Please remove and properly store or dispose of the tires and all other debris, as shown in the attached photos.*"
Item# 2 Code 30-36A(2) Complies No Comply By 06/28/2013 IMMEDIATELY
Description The parking/storing of vehicles on areas not designated for parking or lawns is prohibited.

Summarv A service fee of$20.00 will be charged as a result of this notice. It is the responsibility of every property owner to confirm
compliance with these orders before the compliance date(s)as specified. Please contact this office on or before the scheduled
due date(s)to schedule a re-inspection of the property. If you do not contact this office to verify compliance your property will
be re-inspected after June 28,2013. If it is found that you are still not in compliance you will be issued a service fee of$50.00
and the violation will still be required to be corrected.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 6/28/2013
Office hours are Monday through Friday 7:30 a.m.-4:30 p.m.or by appointment.To schedule inspections please call the Inspection
Request ine at 23, -5 oting the address, permit number(when applicable),and the nature of what needs to be inspected.
