Introduction An inspection of your property upon receiving a complaint revealed the following reoccurring violations of the Oshkosh
Municipal Code,which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 16-33A(7) Complies No Comply By 04/17/2013 IMMEDIATELY
Description All dwelling units,vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements)are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants concerning smoke detectors. "'At inspection there was no battery in the smoke detector. Install a battery :
immediately and test the detector to ensure it is working.
Item# 2 Code MUN 16-34A(3)(a) Complies No Comply By 04/17/2013 IMMEDIATELY
Description Every outlet and fiuture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. **At inspection the cover plate for the bathroom switch and
receptacle was missing. Install a new cover plate.
Item# 3 Code MUN 16-35A(3) Complies No Comply By 04l17/2013 IMMEDIATELY
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures,shall
be repaired. **At inspection the kitchen sink was dripping. Make a repair so that it will not leak out.
Item# 4 Code 16-37A(5) Complies No Comply By 04/17/2013 IMMEDIATELY
Description Every owner shall be responsible for extermination of any insects, rodents or other pests whenever infestation occurs in more
than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit. **The
tenant complained that she is seeing cockroaches at night. No live cockroaches were found at inspection but bait was found in
several areas. A report is needed from an exterminator stating that the cockroach problem has been eliminated. There have
been cockroach problems with this complex going back to 2011.

Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
*`Once the cockroach problem is eliminated remove all cockroach bait left behind from the dwelling unit.

and/or occupancy. Upon completing the corrections,the owner/contractoNagent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 5/10/2013
