Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions '

� Item# 1 Code 16-33A(7) Complies No Comply By 05/22/2013 IMMEDIATELY
Description All dwelling units,vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements)are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants conceming smoke detectors. *"Batteries missing from the smoke detectors.
Item# 2 Code ss 101.647 Complies No Comply By 05/22/2013 IMMEDIATELY
Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor
level except the attic, garage, or storage area of each dwelling unit. Except as provided under par. (b)the occupant of the
dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that
the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice, any
maintenance necessary to make that carbon monoxide detector functional. **Detector in basement not working.
�Item# 3 Code 16-35A(3) Complies No Comply By 05/22/2013 IMMEDIATELY
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be repaired. *"Toilet running constantly. Leaks into the room below. Make repairs so the toilet shuts off and drains without
leaking.
�Item# 4 Code 16-35A(1)(a) Complies No Comply By O6/14/2013
Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
*"Repair the water damaged ceiling in the dining room. Repair the torn screen on the patio door. Replace broken ceramic tiles
in the kitchen. Finish patching the wall and paint to finish. Install a transition strip over the tile floor to the patio door so the
area does not accumulate debris. Install the baseboard to the wall in the kitchen. Remove the water damaged flooring in the
upstairs bathroom. Install a new flooring that is water resistant. Patch in the rotted area of flooring under the toilet.
Item# 5 Code 16-34A(1) Complies No Comply By 06/14/2013
Description Every dwelling unit shall have heating facilities which are capable of safely and adequately heating all habitable rooms and
bathrooms to a temperature of at least 67 degrees F with an outside temperature of minus 15 degrees without forcing the
heating system. All habitable rooms to be supplied with adequate heat sources. All heating shall be in operation to maintain
this minimum during all occupied periods. ""Thermostat not working-fan ran constantly on the furnace. Only one supply found '
upstairs in the hallway. Install ducting to the bedrooms and bathroom or install alternate heat source(baseboard).

Description Stairways in one and two family dwellings shall be in sound condition, equipped with proper handrails&guardrails installed in
accordance with SPS 321.04 , safe to use and shall be equipped with treads and risers reasonably uniform. *'Bottom section
of the basement stairs is rotting and flexes when walking across. Make a structural repair to the stairs.

Description Every bathroom and toilet compartment shall have at least one(1)window directly openable to the outside or shall be
equipped with mechanical exhaust ventilation directly to the outside and installed according to accepted engineering practices
or manufacturers written specifications. **Bathroom fan not working.

Item# 8 Code 16-33A(9) Complies No Comply By 06/14/2013
Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in
Section 16-22 OMC. *'Side door is in poor condition. Deadbolt lock is not operating properly. Duct tape has been used to
secure the window panes. Wood panel is cracked.
Item# 9 Code 16-34A(3)(a) Complies No Comply By 05/22/2013 IMMEDIATELY
Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. **Install light bulbs in any open sockets.
Summarv A court date for allowing code violations on your rental property after having received prior notices for the same violations on
this or other rental property has been scheduled for June 12,2013 at 9:OOam, Rm. 194, 420 Jackson St. To avoid further
enforcement action correct the violation(s) by the noted compliance dates and schedule a re-inspection so that compliance
may be verified.
�olations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and retum it to the Inspection Services Division by the Compliance Date of 6/14/2013
