Dear Sirs, PLAN ID # D9-504-0513-P
Examination of the plumbing plans and specifications for this project has been completed. In
accord with Chapter 145, Wisconsin Statute, and SPS 381 through 385, Wisconsin
Administrative Code, the plumbing plans and specifications are approved contingent upon
compliance with the stipulation(s) noted below.
1. SPS 382.40(5)(b)(3) Water distribution piping conveying circulated water or served by a
self-regulating electric heating cable shall be insulated to limit the heat loss. If a circulation
system is used to maintain the temperature, no uncirculated hot water distribution piping
may exceed 25 feet in developed length.
2. SPS 384.10(2)(c) If, upon review, the department determines that a product conforms to
the provisions of chs. SPS 382, 383 and this chapter and ch. 145, Stats., the department
shall issue an approval in writing. The department may impose specific conditions in
granting an approval. Violations of the conditions under which an approval is granted shall
constitute a violation of this chapter. ;
3. SPS 382.31(3) Vents. Every trap and trapped plumbing fixture shall be provided with an
Individual vent, except as otherwise permitted in this chapter. Vents and venting systems
shall be designed and installed so that the water seal of a trap shall be subject to a
maximum pneumatic pressure differential equal to one inch of water column.
4. SPS 382.20(4)(b)(3)(a) Plans proposing the installation of a building sewer for new
construction which is to discharge to a municipal treatment facility shall:
a. Be accompanied by a letter from either the appropriate designated management agency
or sanitary district indicating conformance with an approved areawide water quality
management plan.
5. SPS 382.20(6) Evidence of approval. The plumber responsible for the installation of the
plumbing shall keep at the construction site at least one set of plans bearing the
department's or the agent municipality's stamp of approval and at least one copy of ,
specifications. The plans and specifications shall be open to inspection by an authorized
representative of the department.
6. STATE STATUE 145.06(1)(a) No person may engage in or work at plumbing in the
state unless licensed to do so by the department. A master plumber may work as a
journeyman. No person may act as a plumbing apprentice or pipe layer unless registered
with the department.
In the event installation of this plumbing system has not commenced within finro years from this
date, this approval shall become void. A new application accompanied by full examination
fees shall be filed and an updated approval received before work may commence.
In granting this approval, the City of Oshkosh or its representative does not hold itself liable for
any defects in plans or specifications, plan omissions, examination oversight, construction or
any damage that may result in or after installation. The City of Oshkosh reserves the right to
order changes or additions should conditions arise making this necessary. It shall be
necessary for the installing plumber to obtain a plumbing permit from the City of Oshkosh
before proceeding with actual installation of this plumbing system or any of its parts.

The specifications and/or plans for this plumbing product have been reviewed and determined to be in compliance
with chapters SPS 382 through 384,Wisconsin Administrative Code, and Chapters 145 and 160, Wisconsin
Statutes.
The Department hereby issues an approval based on the Wisconsin Statutes and the Wisconsin Administrative
Code. This approval is valid until the end of May 2018.
This approval supersedes the approval issued on 1/25/2008 under product file number 2008839.
This approval letter shall be incorporated with your previously approved plans and/or specifications approved under
product file number 2008839.
This approval is contingent upon compliance with the following stipulation(s):
• This tank must be designed to withstand the pressures to which it will be subjected.
• The manufacturer must keep at the manufacturing plant a set of plans and specifications bearing the
departmenYs stamp of approval. The plans and specifications must be open to inspection by an authorized
representative of the department.
• When this product receives wastewater from dwellings and is used as a septic tank, it will produce an effluent
quality with a maximum monthly average value for BOD5 greater than 30 mg/L but less than or equal to 220
mg/L, TSS greater than 30 mg/L but less than or equal to 150 mg/L and F.O.G. less than or equal to 30 mg/L. '
• This product is approved to use the following:
-Side opening for when product is used as a holding tank.
- Department approved effluent filter designed to be installed in a four inch diameter tee with extension and
installed in accordance with the product approval for the filter including a properly sized and located access
opening for service and maintenance.
- Four inch discharge opening in riser.
-Bottom and side pipe openings for siphon, pump and holding tanks.

- Manhole riser and cover having a rectangular, round, or square shape with a minimum 24"inside dimension.
-A slide-in dividing wall (Super Screen) in the septic tank or in the first septic compartment of a two
compartment tank. The dividing wall has an 8"diameter hole that has its center located 12"above the bottom
and is centered in the width of the dividing wall. The top of the dividing wall extends at least 4"above the liquid
level and terminates at least 2"below the bottom of the tank cover. The location of the dividing wall in a single
compartment tank is between 1/2 to 3/4 of the inside length of the tank when measured from the tank inlet. The
location of the dividing wall is between 1/4 to 1/2 of the inside length of the first compartment of a two-
compartment tank in relationship to the length of the compartment when measured from the tank inlet.
As of May 15, 2008, a copy of a successful water tightness test report for this product must be AVAILABLE FOR
INSPECTION AT THE MANUFACTURER'S PLANT prior to this product being used as a POWTS holding or
treatment tank in Wisconsin.
The department is in no way endorsing this product or any advertising, and is not responsible for any situation which
may result from its use.
