Introduction An inspection of your property during inspections in the neighborhood revealed the following violations of the Oshkosh
Municipal Code,which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code 17-36B&30-36a(2) Complies No Comply By 05/02/2013 IMMEDIATELY
Description No owner,agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance,eyesore and/or hazard.There shall be no parking in designated setback areas or outside of designated parking
spaces, except in the driveways of single-family and two-family dwellings. "At inspection a vehicle was found parked in the
side yard. Vehicles are only allowed to be parked/stored in the designated parking araes(driveway or garage). A citation has
been issued to the owner of the vehicle with plate#678 DNW. Move the vehicle immediately to the driveway.

nuisance, eyesore and/or hazard. **The rutted up yard shall be leveled back out to include additional topsoil if necessary and
reseeded with turfgrass.
Summarv A service fee of$20.00 will be charged as a result of this notice. The fee will be invoiced under a separate mailing from the
City of Oshkosh. Failure to pay the assessed service fee will result in fee being added as a special assessment on your
property tax bill. Correct the violation immediately and contact me at(920)236-5137 to verify compliance. If you fail to contact
me a re-inspection will be made after 5/2/13 to verify compliance. The court date for the owner of the vehicle is May 8,2013 at
9:OOam Rm. 194,420 Jackson St.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and retum it to the Inspection Services Division by the Compliance Date of 5/2/2013
