Introduction Having had no further contact or confirmation of compliance concerning this order,an inspection was made on 4/23/13 to
determine the status of the corrections. The following uncorrected/reoccurring violations were noted:
Item# 1 Code 17-366 Complies No Comply By 04/22/2013 IMMEDIATELY
Description No owner,agent or occupant shall allow on any residential premises any junk,debris or other condition,which creates a public
nuisance,eyesore and/or hazard.This includes the proper storage&removal of garbage,debris and yard waste. "*At
inspection loose garbage and recyclables were found in the yard and the garbage cart for one of the units was overflowing and
bags of garbage were being stored on the ground. Clean up all loose debris and have your tenant put their garbage out for
regular collection.
Item# 2 Code 23-9A Complies No Comply By 04/22/2013 IMMEDIATELY
Description All garbage shall be kept in carts issued by the Sanitation Division. **At inspection bags of garbage were being stored on the
ground behind one of the units. All garbage and recyclables are requried to be stored in their designated carts and put our
regularly for collection.
Summarv A service fee of$50.00 will be charged as a result of this notice. The fee will be invoiced under a separate mailing from the
City of Oshkosh. Failure to pay the assessed service fee will result in fee being added as a special assessment on your
property tax bill. Correct the violation immediately and contact me at(920)236-5137 to verify compliance. If you fail to contact
me a re-inspection will be made after 5/2/13 to verify compliance. If you are found to still be in violation an additional service
fee and citation will be issued.

and/or occupancy. Upon completing the corrections,the owneNcontractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 4/22/2013

Summarv It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property. If you
do not contact this office to verify compliance your property willl be re-inspected after 4/22/13. If it is found that you are still not
in compliance you will be issued a service fee of$50.00 and the violation will still be required to be corrected.
