Introduction Our office has received notification that this unit has been modified into two units instead of the intent of the permit that was
issued. The permit that was issued allowed for the combination of both units E and F into one large unit. This eliminated unit
F and made this building into a five unit building. A portion of this building can not be rented due to the fact that the fire
seperation has been eliminated between the units where the doors were cut in.
Item# 1 Code IBC 707 Complies No Comply By 05/15/2013
Description Fire barriers would be required to be installed between two seperate living units. The fire barriers between the units have been
removed thus combining both E and F into one living unit.
Item# 2 Code MUN 7-8 Complies No Comply By 05/15/2013
Description The permit that was issued states: "Alterations to combine units#E&#F into one large unit. A storage area will be
constructed in the basement and penetrations will be created in the firewalls between the two existing units. A seperate
electrical permit will be required for interconnecting the smoke alarms. An additional permit will be required for seperating
these two units in the future to maintain the fire sepertation and to seperate the smoke detection system." Any modifications to
this permit would void this permit. Additional plans and approvals would be required for any modifications not included in the
permit.
Summary If a portion of this unit is being rented out our office will have no other choice than to void the permit that was obtained by
Midwest General Contractors. Please contact me to discuss this situation within the next 10 days at 920-236-5036.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 5/15/2013
