Introduction Having had no further contact or confirmation of compliance concerning this order, an inspection was made on 2/28/13 to
determine the status of the corrections. The following uncorrected/reoccurring violations were noted:

Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a pubiic
nuisance,eyesore and/or hazard. This includes the proper storage&removal of garbage, debris and yard waste. "`Remove
the Ns and all other junk&debris from the yard.
At re-inspection on 2/,t8/13 there was still a N behind the garage on the alley side and garbage/debris on the side of the
house. Remove all junk, debris, garbage,etc.from the yard.
Summarv A service fee of$50.00 will be charged as a result of this notice. The fee will be invoiced under a separate mailing from the
City of Oshkosh. Failure to pay the assessed service fee will result in fee being added as a special assessment on your
property tax bill. Correct the violation immediately and contact me at(920)236-5137 to verify compliance. If you fail to contact
me a re-inspection will be made after 3/14/13 to verify compliance. If you are found to still be in violation an additional service
fee and citation will be issued.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 2/4/2013
