Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code 16-34A(3)(a)&NEC Complies No Comply By 12/21/2011
Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Contact an electrician to correct the wiring to the dryer. Install
a cover plate where missing.
Item# 2 Code MUN 16-35A(1)(a) Complies No Comply By 12/21/2011
Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Add in framing to properly support the weight of the water heater. Leak in living room ceiling was reported to be repaired.

Description No person shall perform or permit the performance of any plumbing work without properly obtaining a permit. A licensed
plumber is required for all work to be done in rental units. No permit on file for replacing the water heater.
Item# 4 Code ss 101.647 Complies No Comply By 12/21/2011
Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor
level except the attic, garage, or storage area of each dwelling unit. Except as proveded under par. (b)the occupant of the
dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that
the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice, any
maintenance necessary to make that carbon monoxide detector functional.

Description There shall be no parking in designated setback areas or outside of designated parking spaces, except in the driveways of
single-family and two-family dwellings. All parking for the dwelling is limited to the concrete driveway area. (See item#7).

Description Seeding of lots: All lots in which a building or structure has been removed shall be seeded with common horticulturally
accepted cool weather perennial grasses consisting of Kentucky blue grass, perennial rye grass and fescues. **A house was
razed that was next to 531 Merritt and the lot has not been established with grass yet. This can be addressed in the Spring of
2012. If you would like to check into your options for expanding the parking area please contact me and I can get you the
details.
Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s) as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 1/13/2012

Chris upon inspection of your premises on 12-5-201 I im a little dismayed with your texts about heat issues
.upon checking the thermostat that was set at 70 degrees about a low of 20 the night before,the unit was
incredibly comfortable enveloping the space with that great radiant heat although the back room by the
stairs was a little cooler it was still very comfortable.the door was closed I suggest u might leave it Open if •
u want it to be as warm as the rest of the unit so the heat can circulate evenly. Recently,on the 3'd of
December you sent me a very ugly text,at about I 1:40 pm I did not receive that till I woke Sunday.
morning. 1 responded that morning and came out Monday to see what the Issue was.The rubber membrane
roof had released from 1 side causing the leak inside the house.The leak was very minor to the space of
which I have pictures of;The roof has been fixed as of 12-5-2011 . Also you express concern over a
washing machine that the cold water inlet is not releasing.When it was installed both hot and cold worked
.you have been in the unit 4 months. I will take a look at it on the 6`h when im there." PLEASE READ
ITEM 4 20 ON YOUR LEASE". There is all of a sudden an electrical issue,u talked to me about 5 days
ago You stated that the specific outlets are not working.they were when you moved in. I have no idea
what possibly could have caused the outlets to stop working they have been active in the building for years
.This I will also look at on the 6th of dec. I would appreciate it in the future if you would not refer to the
lower unit woman's children"as the spawn of her loose loins"as u did in a text . Please note that the texts
you have sent are saved and recorded . fhe unit is a very nice unit . I have taken the liberty to have an
officer of the Oshkosh police department walk thru the unit with me to assess it. This will happen on the
6'h of dec.also. I did this to have verification as to the condition of the unit. Also please read number 20
on your lease so u have a,greater understanding of your obligations as a tenant.
