Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 16-33A(9) Complies No Comply By 11/09/2011
Description All living unit entryways shall be equipped with solid core doors in good condition and deadbolt locks installed as defined in
Section 16-22 OMC. Install deadbolts on both front and back doors.
Item# 2 Code MUN 16-35A(3) Complies No Comply By 11/09/2011
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired. Water heater leaking through side spigot.
Item# 3 Code NEC 400.8 Complies No Comply By 11/09/2011
Description Flexible cords and cables shall not be used for the following: 1)As a substitute for the fixed wiring of a structure 2)Where run
through holes in walls, structural ceilings, suspended ceilings, dropped ceilings, or floors 3)Where run through doorways,
windows, of similar openings 4)Where attached to building surfaces. Tenant was notified to discontinue the use of the
extension cord as a permanent power source.

Description Every floor, floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Make repairs to the storm door(glass is broken in two corners, screen/window are installed wrong), repair the hole in the
bedroom door and repair the hole in the carpeting in the living room.

this office on or befoore confirm the scheduled due date(s)to schedule acre-inspection'of thedproperty.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 12/2/2011
