Item# 1 Code MUN 16-35A(6) Complies No Comply By 10/05/2012 IMMEDIATELY Description No owner or operator shall rent to any other person for occupancy or allow any person to occupy any dwelling unit or premises unless it is safe and clean and complies with all provisions of the Minimum Housing Code and Property Maintenance Code. CITATION ISSUED. At inspection numerous dead and live cockroaches were found in the kitchen. Old bait stations were found throughout the kitchen. Item# 2 Code MUN 16-37A(5) Complies No Comply By 10/05/2012 IMMEDIATELY Description Every owner shall be responsible for extermination of any insects, rodents or other pests whenever infestation occurs in more than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit. Tenants have reported problems with cockroaches, bedbugs and mice in the apartment. Contact an exterminator immediately to treat for all these problems. Dead and live cockroaches were seen at inspection as well as mouse droppings in the kitchen pantry. Tenant has photos of bedbugs he found on one of the upstairs beds. Item# 3 Code MUN 16-35A(3) Complies No Comply By 10/05/2012 IMMEDIATELY Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall be replaced/repaired. Piping to the boiler leaking. Laundry supplies leak and have to have supply shut off to stop the water. Bathroom sink leaking- have to shut off supply to stop the water. Item# 4 Code MUN 16-35A(1)(a) Complies No Comply By 10/28/2012 Description Every floor, floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair. Properly abandon the door(wall in or drywall over)to the jump platform (screwed shut)or remove the screws so the door is useable as an emergency exit. Secure or remove the storm door so that it does not blow around in the wind. Replace the base of the sink base cabinet. Remove the unused hose bibb and seal the penetration into the building. Replace the water damaged ceiling tiles. Repair the laminate end cap in the kitchen. Entrance door does not seal properly-adjust the strike. Item# 5 Code MUN 17-36A Complies No Comply By 10/28/2012 Description All building foundations shall be maintained. Tuck point all areas where mortar is missing or deteriorated. Holes in the basement go clear through to the outside and may be the entrance point for mice. 20150 Page 1 of 2 Item# 6 Code MUN 16-34A(1) Complies No Comply By 10/28/2012 Description All habitable rooms including bathrooms to be supplied with adequate heat sources. All heating shall be in operation to maintain this minimum during all occupied periods. Install a heat source for the bathroom (radiator was removed). Summary A court date for allowing code violations on your rental property after having received prior notices for the same violations on this or other rental property has been scheduled for at October 10, 2012 at 9:00am, Rm. 194, 420 Jackson St Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice and return it to the Inspection Services Division by the Compliance Date of 10/28/2012 Office hours are Monday through Friday 7:30 a.m. -4:30 p.m. or by appointment.
