Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 16-37A(5) Complies No Comply By 12/28/2011
Description Every owner shall be responsible for extermination of any insects, rodents or other pests whenever infestation occurs in more
than one dwelling unit in a dwelling, or in shared of public parts of a dwelling of two or more units or in any vacant unit. There
is a cockroach infestation in apt#4. Cockroaches were found in numerous locations in the apartment. Roaches were also
found in the hall and on the door to the laundry room. Contact an exterminator immediately to treat the building. Contact all
tenants in the building to find out if other units are affected and have those treated as well.
Item# 2 Code MUN 16-35A(3) Complies No Comply By 12/28/2011
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Replace the handle to the hot control in the tub. Tenant stated they
were provided with a pliers to operate the hot water control. Replace the handle immediately. Tenant also stated that the
dishwasher is not working. Have the dishwasher serviced.

Description Every floor, floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
The apartment shall be cleaned including removal of any dead cockroaches,wiping down the walls, ceilings, cabinets,
coutertops, etc., vacuuming and any other measures needed to assist in the elimination of the cockroach infestation.
Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Please contact this office on or before the scheduled due date to avoid further action by the City.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 1/20/2012 -_
