Introduction An inspection of your rental property(living unit)following a tenant complaint revealed the following violations of the Oshkosh
Minimum Housing Code which shall be corrected by the compliance dates specified for each item. If you have any questions

Item# 1 Code MUN 17-36B Complies No Comply By 11/08/2011
Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage& removal of garbage, debris and yard waste. Clean up all
loose garbage&debris from the property. Have the furniture(chairs, mattresses, etc.)that is in the yard disposed of.
Item# 2 Code MUN 16-35A(1)(a) Complies No Comply By 12/01/2011
Description Every floor,floor covering,wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Repair the section of carpeting that is coming loose from the transition strip. Repiar all holes in the drywall. Repair the kitchen
cabinet drawer. Repair the patio screen door so that it operates freely.

Item# 3 Code MUN 16-35A(3) Complies No Comply By 12/01/2011
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Bathroom vanity faucet was leaking through the handle. Also check
the bathrub spout to see if it leaks at the back side(tenant stated that there were leaks).
Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date(s)as
specified. Please contact this office on or before the scheduled due date(s)to schedule a re-inspection of the property.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 12/1/2011
