Introduction During your annual rooming house inspection the following violations of the City's Municipal Code were found which shall be
corrected by the noted compliance date(s):
Item# 1 Code MUN 16-33A(7) Complies No Comply By 09/29/2011
Description The occupant or occupants of a room in a rooming house shall be responsible for ensuring that the smoke detector installed by
the owner or operator is checked periodically and is functioning properly. The owner or operator of every rooming house shall
ensure that a smoke detector is installed in each sleeping room,vacant or occupied, and is in working order. **At inspection
several smoke detectors were tampered with or inoperable. Any future violations will result in immediate citation issuance.
Item# 2 Code SS 101.647 Complies No Comply By 09/29/2011
Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor
level except the attic, garage, or storage area of each dwelling unit. Except as proveded under par. (b)the occupant of the
dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that
the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice, any
maintenance necessary to make that carbon monoxide detector functional.
Item# 3 Code MUN 17-36B Complies No Comply By 09/29/2011
Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition,which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage& removal of garbage, debris and yard waste. Clean up
any debris from the yard. Discontinue dumping your ashes on the ground.
Item# 4 Code MUN 16-35A(3) Complies No Comply By 09/29/2011
Description Cracked/damaged/leaking fixtures or other plumbing conditions,which create un-sanitary conditions or unusable fixtures, shall
be replaced/repaired by a licensed plumber with permit. Leaking water heater is required to be replaced.
Item# 5 Code MUN 16-34A(3)(a) Complies No Comply By 09/29/2011
Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be
connected to the source of the electric power in a safe manner. Discontinue use of extension cords as a permanent source of
power(bedroom closet). Install a cover plate on the switch in the closet and bedroom. Do not use a standard complact
fluorescent bulb where a dimmer is used as a switch (bulb should be dimmable).
Item# 6 Code NEC 352.30 Complies No Comply By 09/29/2011
Description Rigid Nonmetallic Conduit shall be secured within 3'of every box,fitting, and conduit termination, as well as every 5'along the
developed length.
Item# 7 Code MUN 17-36A _ Complies No Comply By 10/22/2011
Description The exterior of every structure or accessory structure including fences shall be maintained by the owner, occupant or person
authorized to use same free of conditions reflective of deterioration and/or inadequate maintenance. OSB cannot be used as a
finished exterior surface (boarded up windows).

Description Every outside stair, every porch, every guardrail and every appurtenance thereto shall be so constructed as to be safe to use
and capable of supporting the load that normal use would cause to be placed thereon, and shall be kept in sound condition &
good repair. Repair the front entrance railing and install a railing on the flight of stairs where missing.
Item# 9 Code MUN 16-35A(1)(d) Complies No Comply By 10/22/2011
Description All interior doors&frames to be maintained in good repair. Replace or remove the damaged door by the kitchen.
Item# 10 Code MUN 7-43 Complies No Comply By 09/29/2011
Description Before commencing construction, installation, alteration or remodeling of any heating,ventilating, or air conditioning system, or
part of a system, a permit shall first be secured. There is no permit on file for the furnace that was replaced. This was noted in
a Correction Notice from your inspection last year. Contact the HVAC contractor who installed the furnace to get the requried
permit.
Item# 11 Code MUN 16-34A(1)(c) Complies No Comply By 10/22/2011
Description All windows in rental dwellings shall be capable of being opened and closed easily and remain open without the use of props.
Windows shall be free of chipped, peeling paint, loose or missing glazing compound, broken panes of glass and be weather
tight. Fix any broken windows.

Description All habitable room windows/door windows shall be effectively screened with screens in good serviceable condition. This
requirement was noted on a Correction Notice from last year's inspection.

Description Every floor, floor covering, wall, ceiling, kitchen counters&tops, cupboards&exterior doors etc shall be kept in good repair.
Repair or replace the sagging section of ceiling tiles by the front entrance.
Summary It is the responsibility of every property owner to confirm compliance with these orders before the compliance date as
specified. Failure to correct the violations or submit your schedule for compliance will result in termination of your Rooming
House License. Please contact me at(920)236-5137 if you have questions. A re-inspection needs to be scheduled to verify
compliance before your license will be renewed.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections,the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 10/22/2011
