Description All dwelling units, vacant or occupied, shall be provided with working, approved smoke detectors one each in the basement
first and second floors. Common area smoke detectors(basements)are the sole responsibility of the property owner. The
owner shall replace batteries once per year or at the beginning of new tenancies. Written responsibilities shall be given to the
occupants concerning smoke detectors.

Item# 2 Code ss 101.647 Complies No Comply By 03/27/2012 IMMEDIATELY
Description The owner of a dwelling shall install a functional carbon monoxide detector in the basement of the dwelling and on each floor
level except the attic, garage, or storage area of each dwelling unit. Except as provided under par. (b)the occupant of the
dwelling unit shall maintain any carbon monoxide detector in that unit. (b) If any occupant gives written notice to the owner that
the carbon monoxide detector is not functional,the owner shall provide,within 5 days after receipt of that notice, any
maintenance necessary to make that carbon monoxide detector functional.

Description No person shall perform or permit the performance of any plumbing work without properly obtaining a permit. A licensed
plumber is required for all work to be done in rental units. A permit was not obtained for replacing the water heater. CITATION
Item# 4 Code NFC 10.10.7 Complies No Comply By 04/03/2012
Description Joints between sections of connector piping and connections to flue collars or draft hood outlets shall be fastened with sheet
metal screws or other approved means. Secure venting to drafthood of water heater. Install screws where missing at venting
joints.

Description A vent connector shall be installed without any dips or sags and shall slope upward toward the vent or chimney at least 1/2 in./
ft. Venting for the water heater pitches down at chimney connection.

Description Every outlet and fixture shall be properly installed and shall be maintained in a good and safe working condition and shall be

Description The exterior of every structure including fences shall be maintained by the owner in good repair. Make repairs to the fence
between the house and garage or remove it.

Introduction On January 12, 2012 you were contacted regarding a tenant complaint and inspection at your property. A list of repairs was
discussed with you. You stated that you would contact this office back in a week with an update on what repairs had been
completed. As of 3/20/12 there has been no contact received from you. The following violations shall be completed by the
noted compliance dates:
