Please read the following information and sign at the bottom to acknowledge tha you have read
and agree to comply with these conditions.
1. All habitable rooms must be provided with 8%light and 3.5%ventilation Bedrooms
located on the second floor or below ground level must be provided with �ress windows
in addition to the light and ventilation requirements. Basement windows e also
required to have window well azeas in accordance with ILHR 21.03.
2. Garage spaces shall be separated from the dwelling unit in accordance wi ILHR 21.08.
The door and frame assembly between the dwelling unit and an attached arage shall be
labeled by an indenendent testin,g a�ency as having a minimuxn fire-resi ve rating of 20
minutes. All drywall joints are required to be taped/sealed unless the join are fitted so
that the gap is no more than a dimes thickness( 1/20-inch)and backed by solid wood or
drywall with the joints staggered.
3. All penetrations in exterior wa11s must be sealed prior to the insulation ' ction. This
includes holes in the top and bottom plates, gaps around heating ducts, an holes in the
return air spaces.
4. Smoke detectors are required on every floor and within 6'of all bedrooms The smoke
detectors are required to be hardwired and interconnected.
5. Hydromassage tubs are required to have an access panel to motor. The a ss panel must
be accessible without removing the finish of the house.
6. All overframing must be designed by the truss manufacturer,according to standards set
by the truss plate institute, or structural calculations must be provided pri r to the rough
building inspection.
7. Truss plans must be submitted and approved prior to framing the roof.
8. Truss plans including the truss layout must be provided on site at the rou building
inspection. In addition to the truss plans,microlam calculations must be rovided on site
at the rough building inspection for all microlams with point loads on the .
9. Calculations must be provided for all cantilevered floors with point loads. Floor trusses
and TJI's must be designed for the cantilevered areas.

; 10. The property owner and applicant is responsible for compliance with ILHR 21.125
(Erosion Control). When installing access drives,a minimum of 2"-3" cliameter stone is
required in addition to a minimum of 6" depth, 30'-50'leng�th, and 14'-20'width. Erosion
control measures must be maintained until the lawn is established.

.� 11. All storm sewer inlets must be protected with approved erosion control devices. Tracking
; (i.e. sediment)must be removed from the street at the end of each workday. Repair any
� siltation or erosion damage to adjoining surfaces and drainageways resulting from land
� development or disturbing activities.
� 12. The building lacard must be osted on site at all times. Once the r

� the placard must be posted on the patio door or on a kitchen window. If the placazd is not
; present at any inspection, stop work orders will be posted. Additional pla.cazds can be
� obtained through our department if the cazds are misplaced.

� 13. All mechanicals must be roughed in prior to requesting the rough building inspection. If
� on the inspection the mechanicals are not roughed in,the inspection will fail and a$100
� re-inspection fee will be charged.

E 14. Attached is a copy of the final inspection checklist. At the final inspection all items on

this list must be completed to avoid the $100 re-inspection fee per inspection.
15. When requesting final inspections please note that a minimum of 2 days notice is needed
from the time the inspection is ready. Futniture is only allowed to be placed in the center
of the gazage floor and a minimum of a 3'pathway must be provided azound the walls of
the gazage. At the final inspection no furniture or personal belongings aze allowed in the
house. Appliances such as the stove,refrigerator,washer,and dryer aze not to be set in
place due to the electrical inspector needing to look at the power serving the appliances.
A 6' stepladder must be on site at the final inspection to provide access to the attic.
16. The building placazd must remain on the patio door or on a kitchen windaw until
occupancy is approved. Occupancy is not allowed prior to the placard having a11 of the
final inspection approved stickers on the backside. If occupancy occurs prior to the
inspections being approved daily citations will be issued with no further notice being
given.
17. All site grading must be done in accordance with the approved drainage plan on file in the
public works office, 3rd floor.
