Scope of Services

ENGINEER will provide the following services to OWNER:
1. Conduct an initial archaeological and historical screening to identify potential sites that
may impact the project. If sites are identified, the results of the screening will be
provided to OWNER.
2. Conduct an initial phase 1 -type screening to identify potential hazardous material (Haz
Mat) sites that may impact the project. The screening will include review of readily
available state and federal databases, review of Sanborn Fire Insurance Maps, and one
site visit to the project location and completion of one Site Summary Form. If Haz Mat
sites are identified, the results of the screening will be provided to OWNER.
OWNER acknowledges that ENGINEER is not, by virtue of this Task Order, the owner
or generator of any waste materials generated as a result of the Hazardous
Materials /Contamination Assessments services performed by ENGINEER under this
Task Order.
3. Conduct sediment sampling in accordance with NR 347 and provide results to OWNER
and Wisconsin Department of Natural Resources (WDNR).
4. Identify wetland impacts and determine compensation for wetland loss, if necessary, in
conjunction with WDNR, following the procedures in TRANS 207 and the Wisconsin
Department of Transportation Wetland Mitigation Banking Technical Guideline dated
March 2002. Wetland mitigation drawings for the project may be provided through an
amendment to this Task Order.
5. Evaluate the potential for discharge of fill materials into the Waters of the United States,
in accordance with the provisions of the Clean Water Act and Chapter TRANS 207, and
prepare the necessary permit application. Preparation of an individual Section 404
permit application for submittal to the United States Army Corp of Engineers may be
provided through an amendment to this Task Order.
6. Prepare and submit a Water Resources Application in accordance with TRANS 207 for
project permits to OWNER and WDNR for review and approval.

7. Obtain existing utility maps from known utilities within the project area and obtain input
from the utility facility owners to establish features of the project that will affect utility
facilities.
8. Assist OWNER in holding one informational meeting at approximately 30 percent
completion to acquaint the public with the concepts of the project.
9. Attend up to four design meetings with OWNER to discuss applicable features of the
project.
10. Conduct topographic survey of project area.
11. Conduct topographic survey of Sawyer Creek from Koeller Street to Sawyer Street.
12. Locate the necessary section corners and provide information for the preparation of plats
and acquisition of rights -of -way and property. Surveys will include ties to two section
corners, quarter section corners, or to street lines or block corners in platted areas. These
ties will be in sufficient detail to permit the preparation of proper legal descriptions of
the lands acquired. Ties to more than two section corners may be provided through an
amendment to this Task Order.
13. Monument the new right -of -way boundaries.
14. Assist OWNER in engaging the services of a geotechnical consultant to perform the
necessary geotechnical investigations and make recommendations on design criteria.
15. Prepare road drawings and submit to OWNER for review. It is anticipated that 250 feet
of approach services will be necessary for the drawings. Additional approach length may
be provided through an amendment to this Task Order.
16. Provide hydrology calculations to be used in the bridge analysis. Submit the calculations
to OWNER and WDNR for review.
17. Provide hydraulic calculations to be used in the bridge analysis consisting of use of the
regulatory model after converting to a HEC -RAS model. Submit the calculations to
OWNER and WDNR. Up to three bridge span options will be modeled.
18. Investigate the need for a CLOMR/LOMR from the Federal Emergency Management
Agency (FEMA) based upon the hydraulic calculations for the proposed bridge. If a
CLOMR/LOMR is necessary it may be provided through an amendment to this Task
Order.
19. Provide hydraulic calculations in the Sawyer Creek Stormwater Plan HEC -RAS model
to assess the feasibility of coupling a larger bridge span with reduced or eliminated
channel improvements (streambank restoration and dredging) upstream and downstream
of the Westfield Street Bridge.
20. Prepare a structure survey report/hydraulic report and submit to OWNER for review,
including copies of the design computations.
21. Provide preliminary structure drawings to OWNER for review.

22. Prepare structure drawings and submit to OWNER for review, including copies of the
design computations.
23. Prepare a Right -of -Way Plat. Include the Right -of -Way Plat information on the Plan and
Profile Sheets.
24. Provide right -of -way descriptions for up to four individual parcels of land to be acquired
as right -of -way for the project. Descriptions will be by metes and bounds or, in the case
of platted property, by suitable reference to the platted data. For all unplatted property,
the descriptions will be referenced to and tied into the pertinent section or quarter section
corners. The descriptions will be tied to the project and will be prepared on conveyance
documents. Current tax records will be used to determine ownership of properties and
premises where a temporary right -of -way for the project is to be acquired.
25. Submit the Right -of -Way Plat and descriptions to OWNER for review and filing.
26. Field- locate and temporarily mark the new right -of -way boundaries in a manner that will
facilitate the appraisal of the affected parcels.
27. Prepare plans, specifications, and estimate for an anticipated delivery date of
April 1, 2011.
OWNER's Responsibilities
1. Assist ENGINEER by placing at ENGINEER's disposal all available information
pertinent to this project including record drawings, existing right -of -way plat and
drawings, horizontal and vertical control survey data, traffic projections, title searches,
section and quarter section location and descriptions, and any other data relative to the
scope of this project.
2. Retain the services of a soils consultant to provide any necessary geotechnical evaluation
and recommendations.
3. Provide right -of -way and easement acquisition services, including appraisals as required
by the Right -of -Way Plat.
Service Elements Not Included
In addition to those identified under the referenced Technical Services Agreement, the following
services are not included in this Task Order. If such services are required, they shall be provided
as noted.
1. CLOMR/LOMR Application to FEMA: Any services of this type shall be provided
through an amendment to this Task Order or through a separate task order.
2. Archaeological Survey: Any services requiring locating archaeological sites shall be
provided through an amendment to this Task Order or through a separate task order.
3. Historical Survey: Any service requiring documentation of historical sites shall be
provided through an amendment to this Task Order or through a separate task order.

Compensation
OWNER shall compensate ENGINEER for Services under this Task Order on an hourly rate
basis plus expenses for a not -to- exceed fee of $115,000.
The not -to- exceed fee will not be exceeded without prior notice to and agreement by OWNER
but may be adjusted for time delays, time extensions, amendments, and changes in the Scope of
Services. Any adjustment will be negotiated based on ENGINEER's increase in costs caused by
delays, extensions, amendments, or changes.
Schedule
Services will begin upon execution of this Task Order, which is anticipated on
November 15, 2010. Services are scheduled for completion on May 15, 2011.
