Introduction Having had no further contact or confirmation of compliance concerning this order, an inspection was made on 913111 to
determine the status of the corrections. The following uncorrected /reoccurring violations were noted:

Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition, which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage & removal of garbage, debris and yard waste. Remove all
junk, debris, garbage, upholstered furniture, etc. from the yard. * *As of 919111 the carpeting /pad or other debris in the yard has
not been removed, Please remove it immediately.
Summary A re- inspection will be made on 9122111 to verify compliance with this order. If it is found that you are still not in compliance, a
citation will be issued. Citations for ordinance violations may be issued at $232.00 per day of violation. Please address the
violation(s) immediately and call (920)236 -5137 to verify compliance with this office.
Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections, the own erlcontractorlagent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 7/29/2011
Office hours are Monday through Friday 7:30 a.m. - 4:30 p.m, or by appointment. To schedule inspections please call the Inspection
