Introduction Following a tenant complaint an inspection was performed at the above- mentioned address. The below- listed violation(s) need
to be corrected.

Description No owner, agent or occupant shall allow on any residential premises any junk, debris or other condition, which creates a public
nuisance, eyesore and/or hazard. This includes the proper storage & removal of garbage & debris. Garbage shall be properly
stored behind and adjacent to the rear of the dwelling farthest away from any public street or sidewalk. - Brush behind garage

Description The exterior of every structure (including fences) shall be maintained by the owner in good repair. All exterior surfaces thereof
be kept painted or covered with exterior siding for purposes of preservation and appearance. All building foundations shall be
maintained. - Deteriorated rear screen door.
Summary Please correct the above violations and request a re- inspection within the next 30 days. Office hours are Monday - Friday

Violations must be corrected and approved by the noted compliance dates of each item. Call for reinspections prior to concealment
and/or occupancy. Upon completing the corrections, the owner/contractor/agent must sign and date at the bottom of this notice
and return it to the Inspection Services Division by the Compliance Date of 9/8/2011
