Introduction Having had no further contact or confirmation of compliance concerning this order, an inspection was made on 3/29/11 to
determine the status of the corrections. The following uncorrected /reoccurring violations were noted:

and /or occupancy. Upon completing the corrections, the owner /contractor /agent must sign and date at the bottom of this'notice
