Nicole here are my notes.
03 -22 -11 clarity care filed for an exemption on this property, walked through the building, since
purchase added grip bars in one bathroom, installed a floor level shower in second bathroom, cut
in a passageway between both units, was a duplex, removed a closet from one bedroom and
installed washer /dryer hook -ups, now a licensed adult daycare facility, chapter 50 license up to 4
adults, exempt for 2011. sfs

The accountant from Clarity Care called in December to verify March 1 is the filing deadline for
exemption request. If you can wait until then we can save a trip should they request and exemption
for this property.
Nicole, they may be converting this to a community based retirement home. Do you need an
update if we find out.

I just saw in CAl2 that you made a note that as of 1/3/11 no exemption has been requested. Should I send
another letter trying to get through? I sent a sale letter in May but they called and said they would be filing for
exemption so they didn't think it paid for me to go through it.

Katrina,
Good morning! I went to do an inspection at the above address yesterday afternoon and I noted that the duplex
has been converted into a single family since they cut a doorway between the two units. The permit was for the
following work "220 Westfield- Installing a handicap ramp (1:12 slope) and a 36" exterior door with a compliant
header. 230 Westfield- Replacing the 32" interior doors with 36" doors, remodeling the existing bathroom and
removing walls to increase the size and installing a handicap ramp (1:12 slope)." The permit did not note that the
units were being combined. I'm thinking that they need to fill out the form in your office but I don't have any
contact info for the owner. Let me know what you think.
