Oshkosh Transit System Daily Tracking Log for ADA Trips Instructions: Definitions: This daily tracking log applies to all ADA trips provided. This includes all yellow and green card
trips that occur during OTS bus service hours. Tracking must be completed daily and submitted monthly to OTS. Please note that it is reasonable to expect brief delays in service throughout
the year. This also occurs on the fixed-route bus side when a vehicle breaks down or other incident interrupts normal service. This data tracking form helps us to track occurrences and
determine if there are on-going issues that show operational patterns or practices, which significantly limit the availability of ADA paratransit service. Full and accurate reporting
will benefit our customers, OTS monitoring, and ultimately your business. Past performance has shown infrequent incidents and no significant issues. This form will document your good
record concerning future occurrences. This helps OTS to document monitoring efforts and show that we we all work together to respond when incidents are reported. Excessively Long Trips
-Occurs when trips are longer than 60 minutes. ADA trips need to be comparable to fixed-route trips (time on bus+transfer time+time walking to and waiting for bus=60 minutes) If one
of the circumstances defined below occurs, take notes in the "Notes" column in the same row of the applicable day. The notes should provide a brief explanation about the occurrence.
For instance, if there is a untimely pick-up, briefly describe why the vehicle was too early or too late. Trip Denial -Includes: (1) any trip scheduled in advance where the scheduled
pick-up time is more than one hour before or after a requested pick-up time, even if the customer accepts the time offered; or (2) when a customer requests a round trip and only one
leg of the trip can be accommodated, if the customer declines the one-way offer, both legs of the trip need to be counted as denied trips. Missed Trips -Occurs when a passenger is waiting
for a scheduled pick-up and the vehicle never arrives or if the vehicle arrives outside of the pick-up window (15 minutes either side of the scheduled pick-up time) and the passenger
has left or does not board. If the vehicle arrives late or early and the rider elects to take the trip, it is considered a late pickup or early pickup. This is an un-timely pick-up and
not a missed trip. Untimely Pick-ups -Occurs when the vehicle arrives before or after the pick-up window (+15/-15) and the rider elects to take the trip
